Brief Summary of Purpose: The SE Licensed Practical Nurse will be an active member of the Summit ElderCare Care Team to ensure participants receive high quality and timely care and support whether at the PACE center or in the community. The SE Licensed Practical Nurse will carry out nursing tasks as assigned by their direct supervisor and in accordance with their license (i.e., receive calls from participants and their caregivers, appropriately triage each call and conduct appropriate follow up including timely documentation in the medical record, administration of prescribed medications to participants attending the PACE center, recording vitals, carrying out treatments and wound care as assigned for participants attending the day center, make frequent home visits in the community to evaluate the home environment, fill weekly medication packs, record vitals, administer prescribed treatments, deliver relevant health teaching for participants and their caregivers, and make recommendations to the IDT regarding unmet participant needs, etc.). Assignments will require occasional or frequent travel throughout the service area to/from participants’ homes and contracted facilities. Job Duties & Essential Functions: Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider. Triage patient phone calls and troubleshoot medication management issues Documents nursing observations, interventions, and patient’s response to treatment in patient records. Maintains and monitors stock inventory in the treatment center and replenishes supplies in exam rooms on a timely basis. Prepares and processes requisitions for routine supplies in accordance with agency procedures. Monitors the integrity of equipment and promptly reports malfunction, missing or damaged equipment to the Registered Nurse (RN) or Office Manager. Demonstrates competency in answering phones, patient registration, making appointments, computer data entry and retrieval of reports. Copies, prepares records, and stores and maintains appropriate records. Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions. Conforms to policies and regulations governing patient rights and confidentiality of information. Adheres to protocol for patient flow (1. Reception, 2. Registration, 3. Labs, 4. Escort to Provider). Overview The Department of Surgery at Macon & Joan Brock Virginia Health Sciences at Old Dominion University is seeking an outstanding Licensed Practical Nurse (LPN) to join the team. The Licensed Practical Nurse (LPN) is responsible for overseeing and assisting with the day-to-day patient care including but not limited to taking vitals, administering meds/treatments, assisting physicians, doing wound care, patient education, documentation, coordinating care, focusing on comfort and supporting quality initiatives, in a fast-paced office. LPN will also assist with training new staff and a point of contact for medical assistants. Responsibilities Triage incoming patients by obtaining vital signs, following standing triage orders, reconcile medication list, reconcile allergies, make sure immunizations are up to date and prepare patients for the visit. Completes delegated tasks from the current and prior day i.e. patient messages, prescription refills, lab results, referrals, home health requests, medication prior authorizations, patient forms, and physician follow-up. All tasks must be reviewed, addressed, and marked appropriately before the closure of each business day. Assist physicians with examinations, minor surgeries, diagnostic procedures and treatments. Provide basic patient care and education as directed by provider and standing orders. Coordinates services with specialists i.e. radiation oncology, pulmonary, endocrinology, nutritional counselling, nephrology, dermatology or social services consultation. Documents all correspondence and interventions in EPIC. Provides patient education and information as instructed by providers and as outlined in the Department’s clinic protocols.
