Position Summary: Provides high quality nursing care for assigned patients in accordance with nursing standards of care and practice under the supervision of a Registered Nurse. Works collaboratively with and under the direction of a Registered Nurse. A. Demonstrates the ability to communicate observations of patient to members of the healthcare team. B. Supports the philosophy and objectives of the nursing department. 2. Assists the Registered Nurse with nursing process. A. Participates in patient data collection, including the admission process. B. Contributes in determining outcomes (goals) that will involve the patient and family. C. Implements plan of care in collaboration with the nursing team. D. Assists RN to evaluate patient’s response to interventions. E. Recommends changes in plan of care to RN and participates in patient care conferences. 3. Demonstrates competence in performance of nursing skills and procedures relative to clinical area and scope of practice. A. Practices within the scope of nursing care consistent with policies and procedures and Pennsylvania licensure for LPNs. B. Partners with the RN to implement evidence‐based interventions and treatments unique to the patient need. C. Assists with health education of patients and family, utilizing patient education resources. D. Prioritizes work assignments and sets goals in order to manage daily patient assignments. 4. Documents in a thorough, concise, timely manner according to the nursing department standards. A. Accurately documents and reports changes in patient's conditions to the RN. 5. Fulfills professional role expectations and requirements. A. Demonstrates behaviors aligned with the organization’s mission and vision. B. Demonstrates positive interpersonal relations in dealing with nursing and inter-professional team to promote efficiency and to provide quality patient care. C. Home Health Licensed Practical Nurse LPN

The LPN is responsible for assessing patient health problems and needs, implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required. ESSENTIAL DUTIES AND RESPONSIBILITIES The essential functions include, but are not limited to the following: Provide skilled nursing care under the direct supervision of the Clinical Supervisor Complete evaluation tasks, including vital signs and medication reviews Coordinate with physicians, therapists and other individuals in the client’s care plan Assess and chart observations of the client’s condition at each visit Maintain complete and accurate documentation of nursing care Work collaboratively with all members of the healthcare team Provide education to clients and families on proper home health care procedures and strategies Record patients' medical information and vital signs. Monitor, record, and report symptoms or changes in patient's conditions. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans Modify patient treatment plans as indicated by patients' responses and conditions Monitor all aspects of patient care, including diet and physical activity Assess the needs of individuals, families, or communities, including assessment of individuals' home or work environments, to identify potential health or safety problems Perform other duties as assigned MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) Associates degree required; Graduate of an accredited state-approved, Bachelor’s Degree from school of nursing preferred Current and valid Connecticut Licensed Practical Nurse license One year of LPN experience, Current CPR certification (required) Driver’s License, Car Insurance, Car Registration Strong interpersonal skills that build personal rapport and establish trust with the client Computer literate, Strong organizational skills Excellent written and communication skills Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers) Home health care experience Bilingual (Spanish) preferred PHYSICAL DEMANDS AND WORK ENVIRONMENT The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
