Responsibilities KEY RESPONSIBILITIES INCLUDE: • Gathers assessment data, monitors comprehensive physiological, psychological, sociocultural, and spiritual needs of patients/significant others under the supervision of the Team Leader. Reports findings to the Team Leader. • Provides competent care consistent with the nursing process and written plan of care, physician’s orders and in accordance with skilled nursing facility policies and procedures. • Participates in admission and discharge planning, in collaboration with health care team, at the time of admission and continues through discharge. • Provides patient and family education to meet their specific needs as outlined in the plan of care and delegated by the Team Leader. • Documents care consistent with guidelines. • Performs other duties consistent with purpose of job as directed.
