Your responsibilities will include: Deliver skilled nursing visits in accordance with the physician's orders and individualized plan of care. Monitor patient conditions and promptly communicate changes to the RN Case Manager and interdisciplinary team. Provide wound care, medication management, treatments, and patient education.
