Collects data and make assessments, develops a working diagnosis, determines interventions and disposition per guidelines. Demonstrates best practices and quality care. Assesses the physical, social, and emotional needs of patients and families. Teaches and counsels patients and families concerning heath assessment and care including prevention of disease and health maintenance. Accurately and completely reports and documents status, care rendered, response to care, provider orders, contacts with patient and/or other healthcare providers. Accurately and completely performs patient follow-up activities including laboratory results and diagnostics, appointments, referrals, medications, and call backs. Able to assist in the ordering /re-ordering of prescriptions within the guidelines of the company protocol. Adheres to all company policies, including but not limited to, OSHA, HIPAA, compliance and Code of Conduct. Regular and dependable attendance. QUALIFICATIONS Education and Experience Required: High school diploma or equivalent. Graduation from an accredited school of vocational nursing. Preferred: Previous experience in an ambulatory or acute care setting preferred. Two (2) or more years of clinical experience preferred (within the last five (5) years). Experience working in the related specialty preferred. Telephone triage experience preferred. Previous experience in an ambulatory, hospital, or home health setting preferred. Bilingual in English and Spanish preferred. Certificate/License: Current licensure as a Licensed Vocational Nurse (LVN) in the State of Texas. The Ideal Canidate will possess a CDC IP certification with experince regarding infection control and knowledge of statewide IC specifics. This postion with be within the nursing managment team, thus serving as a resource by attending to call, assisting staff on the the floor when needed and acting in the capcity of leadership will be necessary. Purpose of your job position The primary purpose of your job position is to plan, organize, develop, and direct all in-service educational programs throughout the facility in accordance with current applicable federal, state, and local standards, guidelines and regulations, and as may be directed by the administrator, to assure that the highest degree of quality resident care can be maintained at all times. Essential Functions: Must be able to plan, develop, direct, conduct, evaluate, and coordinate staff training, education, in service, and orientation Must maintain associate training and in service records Must exhibit excellent customer service and a positive attitude towards patients Must perform proficiently in all competency areas including but not limited to patient rights, and safety and sanitation. Must have expert knowledge in the field of practice. Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department. Must understand and follow company policies including compliance procedures. Must be able to communicate and function productively on an interdisciplinary team. Implement recommendation from the Quality Assessment and Assurance Committee as the relate to the educational needs of this facility. Ensure that all personnel attend and participate in annual OSHA and CDC in-service training programs for hazard communication, TB management , and bloodborne pathogens standard. Education, Experience, and Licensure Requirements: Must be currently licensed/registered in applicable state. must maintain an active nursing (LPN) or (RN) license in good standing throughout employment ; Must have experience in supervision and principles, practice of teaching, etc. Must possess skills in leadership and communication Must demonstrate dependable, regular attendance. Pay Rate: $34-$42
