Manager of In-Home Support Program RN Purpose of Position: Management of a program that employs direct care staff that assist program recipients in their homes, in order to provide those support services that allow recipients to reside in the community of their choice. Provide training to staff regarding therapeutic service provision and supports, following state guidelines for personal care services and waiver services. Develop and provide clinical training of at least 40 hours for each Personal Care Attendant, and clinical training of Direct Care Attendants as required. Evaulate recipient care needs in the recipient’s home, traveling as necessary to regional villages. Facilitate in-home care provision to recipients by medical providers in Nome and the regional villages. Demonstrate personal initiative and accountability while overseeing and managing the department, any program grants, and the department's budget . Forecast needs, allocate resources and/or take corrective action as necessary. Evaluate program effectiveness and provide quality assurance reports. Work with the Personal Care Attendant Office Manager to manage program and staffing needs. Develop and monitor compliance with individualized service plans that address presenting problems and care needs, goals, progress towards those goals, planned services, and supervise the person responsible for each service or action step. Teach recipients and their families to understand conditions, medications, and self-care skills, as appropriate. Coordinate and submit billing for the direct care staff to the appropriate agency. Facilitate family and inter-agency collaboration. Monitor preparedness for client discharge and refer problematic situations to appropriate resources. Resolve critical issues and problem situations as they occur and maintain compliance and correct deficiencies with state surveys Perform other duties as assigned. Assists/oversees restraints or use of seclusion in an emergency situation. Notifying the patient’s physician for orders as soon as possible within the guidelines of Seclusion and Restraint Policy. Reviews policy and procedures on a regular basis. Directs activities in all emergency situations, including fire, internal and external disasters, and environmental hazards (tornados) according to procedures. Reviews intake information as needed and collaborates with the Intake Referral Coordinator to ensure a timely referral process. Investigates unusual incidents/complaints from patients, personnel and other departments and reports to the Assistant Chief Nursing Officer. Preferred Qualifications Role Purpose Under the direction of manager, plans, directs, and coordinates activities of a unit on a designated shift. Performs direct patient care as needed and functions as clinical resource to nursing and support staff. Assesses, plans, evaluates and documents the nursing process of assigned unit as needed. Responsibilities Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into the ambulatory care setting. Plan of care reflects application of expert holistic clinical knowledge and skill to anticipate and prioritize complex care needs, anticipate barriers, incorporating interprofessional team and caregiver/family input, and identifies opportunities to improve patient outcomes. Actively utilizes and champions scientific inquiry for evidence-based practices (e.g., participates in journal club, facilitates practice change based on new knowledge, recommends new standards of care or revisions to current standards, conduct a literature search to support unit or organizational goals, disseminate new knowledge obtained from relevant conference or continuing education). Explore opportunities to advance nursing practice (e.g., support a community service health event as a representative of the healthcare profession, patient/family education group, community advisory board, member of Ethics Advisory Committee, serve as adjunct faculty, leadership board in specialty organization). Analyzes gap in practice standards and provides feedback to peers in collaboration with leadership to enhance safe patient care.
