As a Nurse Practitioner, you will provide types of care, including but not limited to: Be comfortable and proficient with documentation and CPT coding Evaluation of new-admission visits Coordination of discharge visits Monthly stability visits for long term residents Evaluations for patients exhibiting changes in condition Advanced Care Planning Other Duties & Responsibilities Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease Prescribe all necessary medications and treatments Perform comprehensive physical examinations of patients Develop and implement patient management policies and procedures Facilitate referrals to other healthcare professionals and medical facilities Communicate with attending physicians and/or specialists regarding patient care Be comfortable and proficient with documentation and CPT coding Requirements Licensure / Certification / Registration: Advanced Practice Licensure (APN) with an applicable state license Must have an active license and DEA # Previous experience in nursing or other medical fields Minimum one-year experience in a Skilled Nursing setting (preferred) Ability to build rapport with patients and their loved ones Ability to thrive in a fast-paced environment Excellent written and verbal communication skills Strong leadership experience with emphasis on teamwork in a healthcare setting Education Master’s degree required
