Nurse Practitioner, Per Diem

Job Description Interested in a dynamic, autonomous practice setting, working with a highly skilled and collaborative APP team? Consider joining our ambulatory care Cancer Center Infusion APP team. The Cancer Center Infusion APP team is responsible for overseeing the care of 50-75 patients per day (depending on size of unit). We cover five Infusion centers providing treatment for Oncology and non-Oncology patients. The Infusion APPs triage multiple issues in real time and ensure continuity of care through collaboration and communication with physicians/surgeons/APP colleagues as well as other clinical and administrative staff. Ultimately, we ensure timely patient care and treatment to meet goals of care, optimize the patient experience and ensure good patient outcomes. In addition to our infusion areas, we cover our three bed Cancer Center Acute Care (CACC) where we manage higher acuity Oncology patients to avoid unnecessary emergency room visits, hospital admissions and facilitate emergency services and admission when necessary. Qualifications Required Qualifications : Graduation from a California Board of Registered Nursing approved or verified school of nursing with a master’s degree in nursing or a Doctor of Nursing Practice (DNP). Two (2) years of clinical experience as a nurse practitioner in the same specialty and setting. Job Description The General Surgery Nurse Practitioner complements and assists the work of the attending faculty, surgical fellow, and surgical residents to provide integrative inpatient and outpatient care for the surgery services, which include acute care, gastrointestinal, bariatric, colorectal, hepatopancreatobiliary, surgical oncology, breast, plastic, and endocrine surgery. Nurse Practitioner - Pediatric ICU

Job Description UCSF Benioff Children's Hospital is recognized throughout the world as a leader in health care, known for innovation, technology and compassionate care. UCSF has one of the top children's hospitals in the nation, according to a ranking by U.S. News & World Report. Our 183-bed UCSF Benioff Children's Hospital has more than 150 specialists in more than 40 areas of medicine. The Pediatric Intensive Care Unit (PICU) at UCSF Benioff Children's Hospital provides extraordinary high-level care for children who require continuous monitoring. The unit has 20 beds for infants and children suffering from a variety of conditions or recovering from surgery. More than 1,000 children per year are admitted into the intensive care units at UCSF Benioff Children's Hospital arriving via ambulance, helicopter and airplane from throughout the western United States. The Pediatric Transitional Care Unit (TCUP) is a 18-bed unit for infants and children suffering from a variety of conditions or recovering from surgery. The TCU provides care to patients with a moderate to high complexity of disease such as those recovering from solid organ transplantation, titration of anticoagulant therapy and other infusion, and post-operative specialty surgery care (ENT, neurosurgery). Patients requiring respiratory support including BIPAP, CPAP, high flow nasal cannula and patients requiring mechanical ventilation with tracheostomy in place are also routinely admitted to the TCUP. The Nurse Practitioner will act as a full member of the team taking responsibility for the safe delivery of care in collaboration with attending physicians, fellows and residents. This team member will meet productivity expectations as set forth by the Division Chief, performing any duties related to patient care activities that are appropriate and clearly delineated. Nurse Practitioners on the PICU and TCUP Team provide care in rotating 12h shifts. Responsibilities Function % of time Duties Inpatient - Direct Patient Care (may include some or all of the responsibilities listed) 70 Independently (independent APP encounter) or collaboratively (shared physician and APP encounter) evaluates and manages patients. Work Shift: 8 Hour (days) (United States of America) Scheduled Weekly Hours: 40 Job Description Summary: Under supervision and in collaboration with the physician staff, the Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with an emphasis on health maintenance and disease prevention. This position is designed for healthcare professionals, such as nurse practitioners and physician assistants, who possess advanced training and expertise in their respective fields. The primary responsibilities include providing patient care in the clinic setting, performing in-office procedures, and focusing on patient education. Job Requirements, Prerequisites and Essential Functions: Pay Range: $85.95 - $100.27 - $117.50 Essential Functions and Responsibilities: Assess patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, and family history, psychosocial situation, and cultural factors; and performing appropriate physical examination. Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks. Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Instructs patient/family regarding medications and treatments. Educates patients regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs. Communicates appropriate case management information to other professionals and community agencies. Prepares documentation for EMR including updating patient records by posting examination and test results, diagnosis, medications, and treatment. Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures. Acts as consultant/collaborator to clinical team on area of specialty. Complies with patient confidentiality requirements. Promotes patient advocacy. Collaborates with physicians in managing acute and long-term medical needs of patient. Identifies patients and families at risk for specific healthcare problems. Diagnosis and Planning: Formulates diagnoses based on health history, physical examination, and diagnostic tests. Formulates and documents health care plans with patients and their families, including treatment, health maintenance education, and counseling. Establishes, reviews, and documents short and long-term goals in collaboration with each patient. Plans comprehensive patient care with other health care providers Implementation Demonstrates ability to provide appropriate care based on needs of a specific individual, including that individual’s age. Institutes treatment plans based on assessments and interpretations of diagnostic tests and physical findings. Counsels patients according to their individual motivation and compliance. Provides telephone advice and triage of acute and unscheduled patients. Documents all pertinent data and interventions in the patient’s records. Provides refills for routine medications. Institutes appropriate action when reviewing abnormal lab data and diagnostic tests. Consults with other providers, as indicated. Evaluation Evaluates the patient’s understanding of and compliance with health care plan by regular follow-up and reassessment. Re-evaluates short and long-term goals with patients and participating health professionals. Education: Patient and Family Identifies the learning needs of the patients and family members. Assesses the patient’s readiness to learn. Provides individualized health education. Develops teaching goals and objectives that meet the patient’s and family health care needs. Staff Functions as a clinical resource person. Continuity/Coordination: Functions as the health care provider for a specified group of patients. Coordinates patient care activities and health care planning among multiple providers, patients, and their families. Communicates and consults with family, staff and unit personnel prior to and during the patient’s hospitalization. May also participate in discharge planning when appropriate. Promotes ongoing communication with community agencies to facilitate continuity of patient care. Advocacy: Promotes the participation of patients and families in planning their care. Assists patients in obtaining information necessary to make an informed decision and supports patients in decision-making. Informs patients of the Patient Bill of Rights. Provides referrals to community resources when appropriate.
