Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient’s specific situation. Nurse Practitioners provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan. Nurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health. Core Responsibilities: Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Educating patients on their health conditions, care plans, and treatments. Participating in Oak Street Health promotional activities. Conducting home visits as needed. Other duties, as assigned. This role reports to the Center Medical Director and works closely with operational leadership. Overview Oncology Nurse Practitioner/Physician Assistant, Medical Oncology- Outpatient Full Time, 80 Hours Per Pay Period, Day Shift Weekend call requirement four times a year where the APP will round in the hospital (with physician support) on Saturday and Sunday. Position Summary: The Oncology Nurse Practitioner is a highly specialized APRN focused on providing comprehensive cancer care to Covenant Health Oncology Group (CHOG) patients in both the inpatient and outpatient setting. The Oncology Nurse Practitioner (ONP) will provide care to hematology-oncology patients in collaboration with and under the supervision of their supervising physician. As a member of the TOG provider team, the ONP will follow established treatment protocols in their practice. The ONP will be a primary representative of the TOG provider team in the community and amongst our referring partners and will be expected to display professionalism, communicate clearly, and be an excellent collaborator in support of patients. The ONP can expect a dynamic daily clinic where patients may require anything from primary care services to complex chemotherapy side effect management. Responsibilities Triage telephone calls from patients, physicians, hospitals, and home health and hospice agencies requiring medical intervention, collaborating with physician(s) as necessary. Performs evaluation and treats acute patient problems, including but not limited to oncologic emergencies, chemotherapy related side effects, and complications from cancer diagnoses. Consults with physician and other medical specialties as necessary; adjusts processes as indicated Performs history, physical examination, analyzes data for appropriate diagnosis and treatment, initiates plans of care for inpatient and office consults in collaboration with the physician, and follows previously established plans of care for both oncology and hematology patients in both the outpatient and inpatient setting. Identifies health hazards in the patient’s environment, as well as patient risk factors. Identifies and considers the cultural patterns, economic status and patient resources in plan of care. Collaborates with social services, nurse navigation, and dietary services to provide holistic patient care. Provides routine follow-up examinations and makes recommendations for adjustments to plans of care, as appropriate. Refers patient to appropriate services when complexity exceeds the scope of practice of the practitioner. Provides pre-treatment chemotherapy education to patients and their families in the office setting. Provides education to office staff as needed. Assists with preparation, administration, and supervision of chemotherapy as staff and physician coverage may require. Assists with office medical procedures as required. Provides first call coverage and assist with morning or evening hospital rounds as required. Serves as a point-person for the oncology group in the inpatient setting. Collaborates with physician to formulate and review medical protocols. Monitors and ensures accuracy of recording on patient medical records. Documents patient history of present illness, review of systems, physical assessment, and plan of care in the patient’s medical record based off of office visit findings. Responsible for conducting periodic review of policy and procedures. Serves as a resource person for other staff personnel.
