Description Insures the delivery of comprehensive health care in accordance with the philosophy, standards of practice, objectives and policies of the Department of Patient Care Services. Responsible for diagnosis and treatment of common problems related to the treatment, management and rehabilitation of patients with ostomies, wounds and/or continence problems as well as patient education and counseling and education of nursing staff. Responsibilities Roles & Responsibilities: Nurse Practitioner, Wound, Ostomy, Continence Specialist (WOCN) who provides safe, competent, quality care based on nursing theory and research to a designated group of patients. Develop, revise and evaluate standards of care; assist in the selection of supplies, technology and equipment. Develop, provide, and evaluate staff education programs, i.e, orientation, in-services, and continuing education. Conducts health assessment interview focused on wound, ostomy, continence related care, Performs focused physical assessment, Develops plan of care based on the diagnosis and individual client needs, Prescribes medications and treatments as appropriate Works collaboratively with the health care team and makes referrals to other disciplines as indicated. Records, reports and interprets the client’s responses to the plan of care Teaches, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems Serves as a role model and resource person for professional and paraprofessional staff. Makes hospital visits to clients and/or takes call as appropriate Qualifications Requirements: Current NY State Registered Nurse License and Registration, NYS NP license & registration certificate. Master’s Degree in Nursing, Graduate of an accredited Nurse Practitioner program, Graduate of accredited Wound, Ostomy, Continence Educational Program BLS required (American Heart Association) At least two years of nursing experience, Nurse Practitioner experience of a least one year preferred. Description The Assistant Nurse Manager of Palliative Care (Oncology) is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Assists in the validation of staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Nurse practitioners are independent practitioners who diagnose, treat, and prescribe for a patients condition that falls within their specialty area of practice. NOTE: Medical orders, progress notes and treatments plans are not required to be countersigned by Responsibilities CLINICAL PRACTICE 1. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 2. Initiates and orders selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines 3. Has completed a pharmacy component of not less than the required semester hours or the equivalent AND has been instructed in New York State and Federal Laws relating to prescriptions and recordkeeping (3-hour course). c. Has a Collaborative Practice Agreement with a physician and designated protocols if less than 3600 hours experience as a practicing Nurse Practitioner. Practice agreements and practice protocols must be established and maintained in the practice settings of the Nurse Practitioner and collaborating physician where they shall be available to the Department for inspection. 2. Practice Protocols Practice protocols shall identify the area of practice to be performed by the Nurse Practitioner in collaboration with the physician and shall reflect accepted standards of nursing and medical practice. Protocols shall include provisions for case management including diagnosis, treatment, and appropriate recordkeeping by the Nurse Practitioner; and may include such other provisions as are determined by the Nurse Practitioner and collaborating physician to be appropriate. These protocols will be determined by area of specialty. 3. Practice Agreements Practice agreements shall include provisions for the following: a. Patient Assignment Patients are assigned to the Nurse Practitioner or physician according to the needs of the patient. b. Diagnostic Tests Laboratory tests may be ordered according to the initial work-up guidelines found in the reference protocol. Collaboration is not necessary for routine diagnostic procedures. However, consultation and documentation should be completed before more extensive tests are ordered, e.g., nuclear medicine tests, MRI, CT scan, IVP, etc. c. Consultation Nurse Practitioners consult with and refer patients to subspecialty clinics as needed. d. Prescriptive Practice Nurse Practitioners prescribe medications according to the New York State Laws and Regulations for prescription writing and record-keeping (see Appendix). e.
