Manages symptoms/toxicities/health abnormalities related to the intense high dose therapy, stem cell rescue and rehabilitation following rescue in collaboration with transplant physician and/ supervisor. Reports data to company for Transplant related registries and appropriate bodies, International Autologous and Blood and Marrow Transplant Registries. Incorporates company Transplant Network recommendations into clinical practice as assigned. * Specialty Specific Responsibilities For Primary Nurses assigned to a Physician, Additional Responsibilities Include: Manages medication refill requests from pharmacies, documenting details in patient charts. Advocate for patients. Support their empowerment in the care process. Facilitates referrals as part of the care plan, treatment course, or advance care planning, in collaboration with scheduling teams, and as directed by a provider. Acts as the patient liaison between healthcare providers and patients. Assist patients in accessing community resources as needed.
