Systematically assess the health status of individuals and groups and record the related health data: Navigate the electronic medical record Collect data on health/symptoms from a patient. 2. Analyze and interpret recorded data, and make informed clinical judgments and decisions regarding the specific elements of nursing care mandated by a particular situation: Correlate new data and symptoms with the existing medical records Determine the appropriate response level within established guidelines. 3. Collaborate, communicate and cooperate as appropriate with other health care team members to ensure quality and continuity of care. 7. Serve as a patient advocate, within the limits of the law. Department Position Summary: The primary care nurse takes a lead role in the prioritization, coordination and delivery of care by the practice team. The primary care nurse provides both direct and indirect care to patients specific to the guidelines/parameters for the department and demonstrates competency for the department specific population age and developmental stage with a focus on social determinants of health, health promotion, and prevention and management of acute and chronic health conditions.
