Core Responsibilities: Provide competent nursing care by displaying proficiency, and executing job responsibilities in a safe and consistent manner. Respond to incoming telephonic requests, ensuring we are reliable and responsive to our patients’ needs and exceeding expectations Provide clinically competent triage and symptom management to patients who may or may not be physically present. Utilize standardized protocols for medication management, prescription refills and prior authorizations. Conduct thorough and accurate reviews of patient medications and update as needed. Provide comprehensive education and direct patient care, particularly around chronic conditions, such as diabetes and CVP. Actively collaborate and monitor the implementation and progress of the care plan for patients on multiple provider panels. Delegate activities to other clinical team members where appropriate to support patient needs. Form relationships with patients and their caregivers to support preventative care and ED/hospital diversion where appropriate. Create a welcoming and engaging environment to meet the needs of our patients, communities, families and teams, focusing on equity of care - meeting each patient where their needs are. Participate in care team meetings and daily huddles to discuss patient care and clinic operations. Deliver an exceptional patient experience through service, responsiveness and respectful care. Perform point of care testing, procedures and specimen collection as needed; this may include phlebotomy, depending on location. Performs other related duties as assigned. What we're looking for Required Qualifications: Active Registered Nurse (RN) Licensure in good standing with the state of Pennsylvania Active CPR and/or BLS Certification Electronic Medical Record (EMR) experience Previous experience in clinic setting with hands-on patient experience, including basic procedures such as collecting vital and specimen collections for labs, and administering injections and vaccinations. Ability to maintain patient confidentiality and process information in a confidential manner, in addition to adhering to HIPAA regulations and laws. Strong communication and assessment skills, with the ability to assess patients with or without a face-to face interaction. Must have the ability to work independently, with adaptability to prioritize multiple tasks at once. Collaborative attitude, with clear communication skills in order to work with an interdisciplinary team to support patient needs. Essential Responsibilities: May perform nursing care functions such as vital signs, removal and/or application of dressings for assessment by licensed provider. Cleaning or irrigating wound after providers assessment and instructions, shaving and skin preparation, ear washes, NG insertion & lavage, urinary catheter insertion & irrigation. Reinforce standardized self care procedures as directed by licensed provider. Recognizes and responds to acute emergency situation following established procedures. Orients patients to system policies and practices. Provides appropriate instruction sheets based upon provider direction. Reviews instruction sheet with patient. Administers medications, excluding IV medications, as ordered by provider. Observes and documents patients response to medications and therapy. May perform tuberculin, coccidioidin and histoplasmin skin tests, provides such tests are part of a tuberculosis control program. May perform immunizations provides they are based upon written protocols. Completes necessary arrangements or organizes treatment rooms prior to surgical and/or examination procedures by setting up or laying out instruments, drapes and sponges contained in sterile trays and assembling equipment items as required for particular procedures as directed. May assist licensed provider with patient examinations and treatments by positioning, restraining patients, and assisting as directed. Provides assistance in treatment rooms during minor surgical procedures and/or various diagnostic examinations by positioning and draping. May hold clamps or retractors, sponge vascular areas, and pass instruments as specifically directed by the physician. Assures safety of patient during procedures. May prepare and transport items such as medical equipment and laboratory specimens. May clean and disinfect instruments/equipment according to standardized procedures. Follows Universal Precautions. Reports incidents of failure to follow procedures to Department Administrator or designee. Maintains departmental files, performing duties such as pulling file and assembling forms prior to appointment, and filing returned diagnostic reports as directed. Initiates and/or completes diagnostic rest forms per provider order, telephones prescriptions to pharmacies, relays medical information from physician to patient, and relays information from patient to physician. Charts in patients medical record as indicated. May perform lab tests including urine dip-tests, acetone tests, hemoccult tests, tests to determine. PH, protein, PKU. May perform hearing and vision tests, glaucoma tests and visual acuities. Collects, by non-invasive and invasive techniques, and preserves specimens for testing, including urine, and sputum and stool. Participates in Department Quality Assurance activities.
