Job Requirements Capital Region Health Medical Group is seeking an experienced Registered Nurse to join our team as a Surgical Services Coordinator supporting our Bariatric Program. This role combines clinical nursing expertise, patient education, surgical coordination, and administrative support to provide continuity of care throughout the surgical journey. Working closely with physicians and the clinical team, the Surgical Services Coordinator serves as a key resource for patients and families before, during, and after surgery. Support individuals in participating in other CBHC services, E.g., Peer Support, Recovery Coaching, Care Management, Psychiatric Rehabilitation Services, Social Support Opportunities, and Community Recovery Supports. Coordinate service delivery with team members, including ensuring access and securing authorization for services. Collaborate with existing providers and other collaterals and deliver services in accordance with client’s plan of care. Participate in team meetings to ensure effective communication among all disciplines involved in participant’s care. Follow the CBHC participant across the continuum of care to ensure care coordination. Partner with participant, their care team providers, and supports to ensure that care plan and safety plan are implemented as developed and make needed adjustments. Monitor participant’s progress in treatment and assist the clinical team with evaluating the need for continued services.
