This person will be part of our overall hospital Case Management team. Primary Responsibilities: To oversee and coordinate each patient's TCU stay beginning with admission through discharge. Work with all members of the interdisciplinary care team to identify goals, address barriers to discharge and ensure patient receives the optimum rehabilitation services in the most efficient manner. Provide the patient and family information about the rehabilitation process, levels of care, and patient rights/responsibilities. Work within the confines of CMS guidelines and/or Managed Care expectations, able to advocate for patients when possible. Ensure patient is provided with appropriate resources befitting next level of care setting (e.g., list of skilled nurse facilities, available home health agencies, etc.). Maintain appropriate patient and family contact regarding patient progress towards goals, care team discussions, as well as continual review of discharge planning efforts to ensure a safe and timely discharge plan is in place. Address disposition changes in timely manner, remaining proactive in anticipating patient's discharge needs. A Brief Overview There are two units within cardiac intermediate care. These units p rovide intermediate level care to patients with congenital and acquired heart disease as well as those with primary rhythm abnormalities. Patients include medical and surgical cardiac patients with an age range typically spanning newborns to young adults. Units may also care for patients under local subspecialty care or general pediatrics. Nurses will interview and be hired to one of these two units depending on position availability at time of application. The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. About this Job: Job Summary The Assistant Director of Nursing (ADON) provides clinical and operational leadership for the Neuro Progressive Care Unit (PCU), supporting the delivery of high-quality nursing care to complex neurosurgical, neurological, ENT, and laryngectomy patients requiring Medical-Surgical and Intermediate Care levels of care. The ADON partners with nursing leadership and the interdisciplinary team to promote safe patient outcomes, staff development, and evidence-based practice while supporting the management of complex neurological conditions, stroke patients, and patients receiving Tenecteplase (TNK). This role provides oversight of daily unit operations, assists with staff coaching and performance management, supports specialty education and competency development, and ensures adherence to clinical standards, regulatory requirements, and MedStar Health policies. As an integral part of the Comprehensive Stroke Center, the ADON plays a key role in advancing excellence in neurological and progressive care nursing.
