Provide patient and family education on diagnosis, treatment plans, self-care, side-effect management, and discharge instructions; coordinate multidisciplinary resources and community referrals. Monitor changes in resident condition and ensure appropriate documentation and assessments are completed. Maintain accurate and organized MDS-related records. Assist with regulatory compliance and survey readiness. Maintain confidentiality and comply with HIPAA and facility policies. Support quality improvement initiatives and other clinical projects as needed. Qualifications Current RN or LPN license in the Commonwealth of Massachusetts . Previous MDS experience strongly preferred. Skilled nursing or long-term care experience preferred. Knowledge of the CMS MDS assessment process and long-term care regulations. Strong clinical assessment, organizational, and critical-thinking skills. Excellent written and verbal communication skills. Ability to work effectively with an interdisciplinary team. Strong attention to detail and ability to meet deadlines. Experience with electronic medical records preferred. RAC-CT or other MDS certification is a plus.
