HOW YOU'LL WORK: You'll provide skilled nursing care in a patient's home as directed by the attending physician.. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Observe and monitor patient conditions and perform OASIS assessments (If you don’t already know this assessment, we will teach you!) to develop an individualized care plan and adjust as needs change. Minimize Patient Risk: Administers medication as prescribed by the physician and helps decrease re-hospitalizations by prioritizing visits for high-risk patients. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate the patient and family on the disease processes using teach back methods to ensure patient and family understanding. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided. Rules and Regulations: understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Job Title: Administrative Nursing Supervisor Department: University Hospital | Clinical Resources Job Description Scope of Position The Lead Administrative Nursing Supervisor (LANS) supports the Nurse Manager with departmental clinical and administrative operations within The Ohio State University Wexner Medical Center. The (LANS) interacts with the department staff, in partnership with nursing and physician leadership, administrator on-call (AOC) and leadership of the hospital medical personnel, and other team members to achieve optimal patient throughput. Professional leadership or clinical certification preferred or obtained within 1 year of hire. ACLS certification preferred/required after hire. Patient Navigator-RN - Main Radiation Oncology - Full Time - Days

Job Description Patient Navigation refers to individualized assistance offered to patients, their families, and caregivers to help overcome health care system barriers and facilitate timely access to quality medical and psychosocial care from diagnosis through all phases of the disease experience. The primary goals of the Patient Navigator role are to: (1) improve patient outcomes; (2) support, educate, guide and empower the patient / family; (3) bridge gaps in communication and promote timely transition between service providers along the healthcare continuum; (4) provide seamless and efficient patient care; (5) remove barriers to care and minimize fragmentation of care for the patient / family; and (6) provide a consistent contact person and advocate for the patient / family, in order to improve the patient / family experience and satisfaction. Responsibilities Assessment Reviews baseline data related to patient: medical, psychosocial, spiritual, financial, and other patient needs. (This includes review of pathology and test results.) Additional training in Nurse Navigation, Advance Practice, or Nurse Practitioner preferred.
