Job Summary Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Pearland and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable.
