Overview The Registered Nurse requires substantial specialized knowledge, judgement and nursing skills based upon the principles of psychological, biological, physical, and social sciences in the application of the nursing process in the respective departments in which assigned. The RN must collaborate with physicians and multidisciplinary team members; provide physical and psychological support to patients, friends, and families; supervising assigned team members and students. Responsibilities Completes patient assessments, formulates plan of care, oversees implementation of plan of care, evaluate and adjust plan of care utilizing critical thinking skills The performance of other nursing acts which are authorized or limited by the board, and which are consistent either with American Nursing’s Association Standards of Practice or with Standards of Practice established by nationally accepted organizations of registered nurses. Care, counsel health teaching and promotion, utilizes Teach Back methodology as affirmation of comprehension The administration of medication, intravenous therapy and treatment as prescribed by a LIP Supervision, delegation and follow-through to other personnel in the performance of activities relating to nursing care. Mentors and precepts students and orient new staff as assigned Reports up chain of command Clinical Nurse Manager/Nurse Manager/House Nurse Coordinator pertinent information Participates on various teams, committees or performance improvement initiatives Ensures a safe and secure environment for patients, coworkers and self Participates in Discharge Planning and Transition of Care (able to define pt needs) Serves as a patient advocate and escalates patient related concerns timely Timely documents pertinent findings in medical record Assist patients in performing activities of daily living and functions as an active team member Assumes responsibility for personnel professional development Utilizes effective communication skills and active listening, exhibits compassionate and empathetic attitude Practices Hands Off communication utilizing SBAR; Performs Bedside Shift reporting utilizing SBAR (unless contradicted) Functions as a positive influence, role model and adheres to professional code of conduct Respects dignity and confidentiality of all matters relating to patients Refer to Special Addendum and Staff Nurse Position Description Performs other job duties as assigned. Qualifications Bachelor's Degree Preferred Associate's Degree Required Registered Nurse. Required BLS upon hire Required De-Escalation Training within 6 months of hire Required Driver's License required Responsible for preparation of and interpretation of client plans of care and systematic assessments/reassessments. Responsible for performing nursing treatments, procedures and total client care in accordance with established agency andnursing care standards. Responsible for communicating pertinent client assessments/reassessments finding to physician. Responsible for initiating referrals to other health care organizations and community services based on identified client needs andphysician orders. Able to travel to client residences in inclement weather and provide nursing care in varying environmental situations.

Case Manager - Care Management

Responsibilities Coordinates and facilitates interdisciplinary provision of comprehensive, patient-centered, quality health care throughout the continuum for patients with acute and chronic health conditions. Fosters achievement of optimal health care outcomes within accepted standards of care. Serves as an expert resource to the healthcare team regarding the continuum of care, efficient use of resources, Best Practice protocols, team-based care, quality indicators and improvements, and regulatory requirements. Ensures a smooth transition of care between multiple health care environments with planned handoffs. Partners with patients and families in identifying health care issues and barriers to self-care in order to set priorities and engage in appropriate interventions. Demonstrates cultural agility and employs health literacy guidelines to provide education regarding self-management strategies. Utilizes rapid quality improvement cycles to continuously monitor, evaluate, measure, and report progress of interventions and outcomes. Paces the case to assure appropriate and fiscally sound care coordination across the continuum. Qualifications Education: Academic degree in nursing (BSN or higher) preferred. Experience: This position requires extensive knowledge of disease management to include diagnostics, treatment and prognosis, community resources and healthcare reimbursement. Minimum 2 years clinical nursing experience required. Relevant experience in one or more of the following healthcare areas preferred: Coordination of community resources Care management of diverse patient populations Ambulatory Care Knowledge of levels of care throughout the health care continuum to include; inpatient, emergency care, rehab, home health, hospice, long term acute care, SNF, ICF, ALF with an overall understanding of utilization management and resource management. Working knowledge of Care Management models across the continuum. Knowledge/Skills: Knowledge of six core components of case management: Psychosocial aspects Healthcare reimbursement Rehabilitation Healthcare management and delivery Principles of practice i.e. CMS guidelines, Interqual criteria Case Management concepts Excellent organizational skills Health literate oral and written communication skills for effective interaction with all members of the patient’s health care team Knowledge of transitional planning to and from all venues Ability to determine and access appropriate community resources Ability to engage patient/family in discussion of health care goals and decisions with attention to cultural and health literacy implications Ability to adhere to and implement regulations in an effective manner. Must serve as a resource to all team members regarding regulatory issues. Keyboard skills and ability to navigate electronic systems applicable to job functions. Licensure Current applicable state RN licensure. Case management certification preferred. BLS for Healthcare Providers from the American Heart Association required for all employees who perform this job in the state of Oregon. Pay Range USD $54.37 - USD $81.21 /Hr.
