Develops the plan of care provided by the home health aide, RN, and LPN by main communication with the clients/caregivers and physicians to facilitate continuity of care, prioritizing care, identifying goals, providing appropriate teaching materials and information on community resources to clients/caregivers, and coordinating services on behalf of the client/caregiver to ensure continuity of care (i.e., equipment/supplies, pharmacy, medical services, transportation, etc.) Implements the plan of care by working closely with the scheduler/s to assure appropriate caregivers are assigned, updates the plan of care based on assessments and supervisory visits, evaluates effectiveness of the plan of care at recertification, and as needed. Provides supervision of Private Duty RN, LPN, and Home Health Aides to ensure quality and continuity of care and that documentation meets the patients plan of care and needs. Reviews documentation of the supervised disciplines for appropriateness and completion compared to the plan of care. What is Required? Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Additional training and certification may be required for area of focus within first year of employment, including but not limited to, Foot and Nail Care Education Course with a clinical component and demonstrate competence in foot care skills and/or be a Certified Foot and Nail Care Nurse, or Foundations in Faith Community Nursing course.
