As part of the admission process, the Admission RN is responsible for the accurate and efficient completion of all required documentation, including OASIS, HIS, comprehensive assessments, and the development of an initial plan of care. The Admission RN is expected to complete a minimum of two admissions per day (or the equivalent of six visit points). Based on patient and agency needs, the nurse may also be required to perform follow-up visits. Additional compensation is provided for visits exceeding 24 visit points in a 32-hour workweek or 30 visit points in a 40-hour workweek, per the agency’s standard per-visit rates. This role reports directly to the Homecare Clinical Manager. Home Health Admission RN Job Responsibilities: Provide skilled nursing care, including patient admission and discharge, in a home-based, family-centered environment while maintaining complete and accurate patient records in electronic and/or paper formats. Educate, demonstrate, supervise, and assess care provided by peers, Home Health Aides (HHAs), family members, and caregivers, fostering a therapeutic relationship that promotes self-advocacy and independence.
