Admission nurses are responsible for orienting new patients to the hospice program, assisting the patient and caregiver with all the legal paperwork, and completing a thorough nursing assessment. The candidate is required to work 10-hour shifts, 4 days a week (Thursday-Sunday 11AM-9:30PM) Qualifications The candidate must be willing to travel throughout the Bay Area. This position involves regular driving over long distances, so comfort with travel is essential Minimum 1 year of hospice experience, SNF, or hospital experience Ability to establish both hospice eligibility and diagnosis when patient is appropriate Willingness to travel to meet our patients where they live (private homes, assisted living facilities, hospitals, etc.) Reliable transportation, valid driver’s license, and auto insurance Registered Nursing Licensure in the state of California

Eligible for a Sign-On Bonus up to $15,000 The Hospice Registered Nurse plans, organizes, and directs the hospice care of patients in conjunction with the Hospice Interdisciplinary Team. The RN provides direct patient care with an emphasis on palliative care, provides education and support to families and caregivers. Completes an initial assessment of patient and family to determine hospice care needs. Regularly re-evaluates patient/family hospice needs. Develops care plan. Essential Job Functions Completes an initial assessment of patient and family to determine hospice care needs. Provides a complete physical assessment and history of current and previous illness(es). Regularly re-evaluates patient/family hospice needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops a care plan which includes the patient and the family in the planning process. Initiates appropriate palliative nursing procedures. Educates the patient and family (caregiver) in ways to achieve palliative goals. Provides health care instructions to the patient, family (caregiver) as appropriate per assessment and plan. Provides direct patient care. Documents patient and family assessments and interventions. Identifies bereavement and discharge planning needs as part of the care plan development. Prepares clinical notes and updates the primary physician when necessary. Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physicians orders as required. Actively participates with the Interdisciplinary Team to coordinate the care plan. Ensure that arrangements for equipment, medication and other necessary items and services are available. Provides in-service education as requested. Responds to patient care needs and deploys to the patient home timely when receiving calls for assistance. Makes home visit at time of death to pronounce Instructs, supervises and evaluates Hospice Aide care every fourteen days. Job Description: Position Summary: The Hospice Registered Nurse (RN) provides compassionate, patient-centered care to individuals with life-limiting illnesses. This role is responsible for managing a caseload of hospice patients, coordinating care plans, and ensuring quality end-of-life care. The RN will also build and maintain strong relationships with facilities, complete inpatient admissions, and serve as a key liaison between patients, families, and the interdisciplinary team. Additional Information: This position will serve the Fond Du Lac, Sheboygan, Manitowoc and Kewaunee after training. Training will take place in the Appleton, WI area for approximately 8 weeks. This position will require an on-call and holiday rotation. Fleet Vehicle Option after 90 days! (Business and Personal Use Option) KEY ACCOUNTABILITIES: Utilizes the nursing process, evidence-based practice, and specific competencies to assess the physical condition and nursing needs of patients, and develops a plan of care in a collaborative practice with the patient and interprofessional team. Plans for the care needs of the patient in collaboration with the interprofessional team to provide the highest quality of care and clinical outcomes. Demonstrates clinical expertise in the provision of care in the clinical specialty assigned, and performs all functions of the professional clinical nurse (RN), which are age appropriate, developmentally sensitive, and culturally specific.
