The workday includes two home care admissions to Hospice Will go to patients' homes to complete the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, home health aides, and more! What you will need: A current license to practice as a Registered Nurse in the State of New York Associates Degree in Nursing, BSN preferred 1 year experience as an RN required, 2 years RN experience with recent acute and/or hospice experience is preferred A bility to navigate through several EMR platforms Good communication skills to collaborate with community partners and work with an interdisciplinary team. Contract agency nurses. * Supervises trains and evaluates home health aides, homemakers and personal care workers in the patient's home, recommending personnel changes to the administration of Hospice as is necessary. * Determines the need for, and recommends the rental or purchase of medical equipment and/or supplies for patients.Provides on-going evaluation of equipment for defects, repairs or additional needs. * Recommends to the Hospice IDG (Interdisciplinary Group) when physical therapy, occupational therapy, speech therapy, nutritional counseling and respiratory therapy are warranted, as per his/her clinical assessment. * Requests physician written orders for changes in the plan of care as is appropriate. * Reports to and consults with the Hospice IDG regarding patient status. * Participates in on-going orientation and in-service training offered both within and outside the Hospice. * Participates in on-call coverage system on a rotating basis as needed by the Hospice. * Serves as a liaison in conjunction with the IDG between Hospice and in-patient setting in maintaining the Hospice patients Plan of Care while inpatient. * Refers major or recurrent problems to the Hospice supervising nurse. * Participates in the on-going Quality Improvement Program. Performs related duties as required. Physical Demands Constant – Standing, Walking Frequent – stooping, bending, stretching, lifting, pulling, pushing, and turning.Lifts objects from 10 lbs to 150 lbs and pushes objects up to 200 lbs. Occasional – Sitting.Lifts with assistance objects greater than 150 lbs. ADA Essential Functions Job Qualification Graduate from an accredited School of Nursing AND if hired after 9/1/10, enrollment in accredited BSN program within two (2) years of employment AND obtainment of BSN Degree within five (5) years of employment, required.Minimum of two (2) years full-time nursing in Home Care (per DOH 700.2,18 regulation).ORBachelor's Degree in Nursing, required.Minimum of two (2) years nursing experience, required. Nursing experience in Hospice, Oncology, or Community Health, preferred. Current registration and license to practice as a Registered Professional Nurse in New York State.
