This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. As a registered nurse, you’ll be a voice for your patients, all while communicating with everyone involved in the plan of care-the patient, the patient’s family, and the hospice care team. You’ll serve as the patient advocate being able to ensure every patient receives the quality care they deserve. After-Hours Hospice RNs are salaried full-time positions to cover on call after hours from 5pm on Friday to 8am Monday morning. You will be fully supported by our 24/7 RN staffed Triage Department alongside your local hospice team. Essential Functions #nsacg Qualifications A heart to serve patients and families and a passion for providing the best possible care Education: Graduate of an accredited school of nursing with a current state license as a registered nurse Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred) Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
