Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Registered Nurse, Clinical Services Inpatient - Days

OVERVIEW Provides patient care by assessing patients, giving medications and treatments, creating individualized care plans, and documenting care. Works with the healthcare team, educates patients and families, promotes patient safety, and supports quality improvement efforts. SCHEDULE: 24-hours per week SHIFT: Days ESSENTIAL DUTIES AND RESPONSIBILITIES Utilizes the nursing process and collaborates with interdisciplinary team members in caring for patients. Administers prescribed medications and treatments in accordance with approved nursing techniques, policies and procedures, standards of care, and State laws practice act. Documents according to standards of care. Performs assessments, planning, intervention, and evaluation of care. Develops, initiates, and executes a patient plan of care according to the individualized needs of the patient, as prescribed by Physician and or clinic policy including patient and family instruction. Participates in department or unit performance improvement activities, may include shared governance. Utilizes clinical expertise and guidance to other units’ departments in support of the patient’s longitudinal plan of care. Utilizes critical thinking regarding patient safety and patient experience and appropriately escalates to the appropriate chain of command. Participates in the professional nursing continuum process, unit practice activities and decisions and peer review process. EDUCATION and EXPERIENCE Graduate of an accredited nursing program. ADN required upon hire, BSN preferred. May require specific years of applicable experience to qualify (per department guidelines) CERTIFICATION or LICENSURE Licensed as an RN in state of practice Approved CPR/BLS certification required within 90 days Other certification(s) per unit requirements. For example : ACLS, PALS, PEARS, and NRP ADDITIONAL REQUIREMENTS Passing the Driver’s License Check and or Credit Check (for those positions requiring). Passing the WI Caregiver Background Check and or IL Health Care Workers Background Check. Must be able to follow written or oral instructions. OTHER SKILLS AND ABILITIES May include color blind test (per department guidelines) WORK ENVIRONMENT The noise level in the work environment is usually moderate. Work Schedule: 90% FTE, 7:00pm-7:00am You will work at the UW Health Swedish American Hospital in Rockford, IL. Additional components of compensation include impressive shift differentials - $4/ hr evening, $5 hr night, and $4.25/ hr weekend shift differential as applicable for hours worked.
