The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents’ needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally. A Family Connects Home Visiting Nurse provides home visiting to all families with newborns in Chicago. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The Home Visiting Nurse will work in a team environment of clinical and non-clinical staff in order to schedule visits, and support families in the community using the evidence-based Family Connects home visiting protocol. In addition to home visiting responsibilities, the Home Visiting Nurse will support the Prenatal Education Department by teaching a minimum of one in-person prenatal education class per week, as assigned. Oncology RN Care Coordinator

Department: 37327 Advocate Illinois Masonic Medical Center - Cancer Care Center Status: Part time Benefits Eligible: No Hou rs Per Week: 0 Schedule Details/Additional Information: Registry / PRN staff are required to offer availability for a minimum of three (3) shifts per calendar month. Clinic is open Monday-Friday. Not opened on holidays or weekends Pay Range $38.20 - $57.30 Major Responsibilities: Participates in the direct care of patients to promote positive clinical outcomes and documents in the electronic medical record per department standards. 1)Assess, plan, implement and evaluate care of women and men with cancer related health concerns. 2)Interview and assess new patients for physicians. 3)Assists physicians per department standards with clinic visits and procedures if needed. 4)Assists patients and staff in the appointment process to expedite timely appointments. 5)Provides patients with appropriate education related to prevention, diagnosis, tests, surgery, chemotherapy and radiation therapy, etc. 6)Teaches patients about the importance of screenings such as mammograms, colonoscopy, etc. 7)Coordinates testing and expedites results reporting. Utilizes the QCL and other tools in the EMR per department standards. 8)Provides emotional support. 9)Assures continuity of care by interfacing with day surgery, in-patient units and out-patient facilities. 10)Maintains the QCL for the MD and facilitates patient flow through the continuum of care in parthership with the physician and other members of the health care team. Participates in indirect care related to patients to promote patient satisfaction, associate engagement, and physician satisfaction. 1)Follow-up with physicians on all abnormal testing in a timely fashion and documents in the EMR per department standards. 2)Documents all education, assessments and care given in the EMR according to department standards. 3)Identifies and collects appropriate Quality Improvement data. 4)Follow-up with patients in a timely and professional manner and documents in the EMR per department standards. 5)Acts as a resource for the hospital in promoting cancer awareness, education and screening tools. 6)Acts as a resource for patients and other employees. 7)Uses effective listening and questioning techniques in a professional and empathetic manner, demonstrating sensitivity to patients and families. 8)Develops educational tools for patients. Participates in Community Education to promote growth. 1)Identifies, develops, and participates in all hospital and community programs related to cancer screening, diagnosis, and cancer related health issues. 2)Develops education programs for cancer screenings programs and awareness. 3)Participates in AIMMC Cancer Committee activities. 4)Under the direction of the Radiation Oncology Coordinator and Cancer Center leadership, works with all hospital departments to provide seamless care for patients recently diagnosed with cancer. 5)Teaches other hospital departments and employees about cancer education, screening tools and treatment options. 6)Develops educational programs for the public. 7)Works with the Wellneses Communities of the Chicagoland area to support their efforts. Engages in professional activities to stay abreast of industry innovations and standards to assist the center with new programming to fund our future. 1)Responsible for continued self-growth and education in areas related to changes in cancer detection, education, and treatment options. 2)Presents a professional image and courteous manner representing AIMMC to the public. 3)Participates in unit and hospital activities to promote service excellence. 4)Promotes team behaviors and acts as a role model for others. Licensure: Nurse, Registered (RN) CPR certification. OCN preferred Education/Experience Required: Effective January 1, 2012: B.S.N., typically required 3-5 years focused experience in clinical areas. • Board Certification by 12/31/2015 or within two years of hire Education requirements if hired into this position prior to January 1, 2012: Graduate of accredited school of nursing and/or B.S.N. or B.S. in health-related field. Knowledge, Skills & Abilities Required: Good communication. Ability to access, plan, intervene, and evaluate the physical, emotional and educational needs of patients. Knowledge of computer operations and applications. Physical Requirements and Working Conditions: Ability to work under stressful conditions and in difficult situations. Ability to work with patients in a fast-paced environment with established time constraints. If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
