Responsible for the efficient collection of meaningful clinical and utilization data to effectively support care coordination initiatives. I. Major Responsibilities: 1. Performs assessment of patient’s physical, functional, and psycho-social needs through the use of screening tools, direct patient and family communication, medical record review, and discussions with other care givers providing services to the patient. Identifies complex patients in need of intensive case management based on established clinical and financial criteria. 2. Performs admission and concurrent reviews as assigned. Performs inpatient reviews in accordance with payer requirements utilizing clinical criteria sets. Meets with 3rd party case managers to discuss cases and assures timely communication of relevant clinical information to third party payers as required. Educates practitioners on the level of care and documentation requirements. 3. Collaboratively facilitates the care of patients throughout the continuum of care. Works as a team member to support /collaborate with Care Coordination Team for monitoring level of care, and for identification of trends in clinical practices and opportunities for improvement. 4. Works with patients and families to establish objectives, goals and an action plan to meet the patient’s individual needs within the indicated level of care. Collaborates with the physicians, other care providers and community resources in the care planning process. 5. Contacts third party payer to clarify benefit eligibility, coverage, and available resources for patients. May negotiate conversion of benefits for previously non-covered services. Researches alternate sources of funding for non reimbursed services that are part of the patient’s treatment plan. 6. Coordinates, implements, monitors and evaluates outcomes of the discharge plan. Evaluates progress of this plan with the discharge planner, patient, family and the patient’s health care insurer. Goals, interventions and time frames are revised as needed as a means of assisting the patient to achieve his/her optimal level of health and functionality 7. Continually evaluates the patient’s level of care. Educates the patient, family and care team regarding resource options, facilitates decision making, initiates plans, and secures resources for the patient on an ongoing basis in complex cases. 8. Participates in quality improvement on a system level through identification of opportunities for improvement, collection and analysis of data, and participation in the development and application of revised processes. Supports departmental and medical center-wide performance improvement programs such as implementation and monitoring of clinical practice guidelines. 9. Fosters compliance with regulatory requirements, good practice, and patient care. Standard Staffing Level Responsibilities: 1. Job Summary: Operating Room (OR) RN is a valuable member of Perioperative and Surgical team. Responsibilities of the OR RN include, but are not limited to, circulating and scrubbing in surgical cases, monitoring patients and administering moderate sedation in select cases per Heywood Hospital’s moderate sedation policy. OR RN takes call per operating room rotation as required by the operational needs. Responsibilities Essential Functions Conducts interactions with everyone in a friendly, courteous and respectful manner. Goes out of his/her way to offer assistance to others. If he/she cannot offer assistance, then finds someone who can. Advocates to ensure privacy and confidentiality while helping others to maintain awareness. Maintains a clean and safe hospital. Responds appropriately and immediately in emergency situations. Maintains a safe and healthy environment for patients, visitors, and/or staff to provide the highest level of physical comfort, and minimize risk and injury. Ensures compliance with regulations to maintain accreditation and licensure. Complies with the Hospital Attendance and Tardiness Policy. Completed all Hospital and Department Specific Mandatory requirements in the prior calendar year. Reports Incidents in a timely and effective manner. All employees with direct patient contact are responsible for actively engaging in the practice of, monitoring and enforcing compliance to hand hygiene. Assesses (and reassesses), plans, implements, and evaluates nursing interventions/plans of care/patient goals. Provides patient care based on standards of care, standards of practice, protocols, and established policies and procedures. Respects each patient as an individual; provides care that fosters the patient’s sense of dignity and positive self-regard. Provides patient/family teaching based on identified learning needs, readiness to learn, and barriers to learning. Collaboratively plans and prepares patient/family for discharge incorporating support systems/resources as necessary. Appropriately delegates and supervises nursing interventions performed by others. Appropriately prioritizes patient care (e.g. seeks assistance as needed from appropriate resources, reassigns personnel as necessary, recognizes need for emergency intervention, etc.). Provides thorough, timely, and accurate documentation of all pertinent data, therapeutic interventions and patient responses according to established standards. Provides timely and thorough report on assigned patients to other health care team members to ensure integration of services and continuity of patient care. Promotes learning for colleagues and self (e.g. functions as preceptor, instructor, trainer, attends professional development programs, etc.). Proactively identifies risk to patient safety and identifies opportunities to reduce medical/health care errors (i.e. appropriate reporting of errors/incidents). Identifies ethical issues in care and develops plan to address. Safely administers medications and intravenous therapy in accordance with physician’s orders, protocols, policies and procedures. Assesses patient pain level upon admission and as necessary, depending upon diagnosis utilizing the numerical pain scale. Reassesses effectiveness of pain control and notifies physicians as needed, as well as, documents assessment and patients’ response to medication. Responsible for obtaining supporting information and completing required documentation related to medication reconciliation for use by the Attending MD. Collaborates with surgeon, anesthesiologist, clinical coordinator and surgical technicians to assure the patients’ well being during surgical intervention. Maintains sterile fields and directs the correct care, handling, and sterilization of sterile supplies and instruments. Identifies patients correctly by checking patient identification bands and consent forms as well as verifying operative procedure and site before taking patients to the operating room. Applies principles of aseptic practice, oversees the selection and preparation of supplies, equipment and instrumentation to all surgical cases. Applies principles of body mechanics and adapts them to various situations during positioning and transfer of patients. Assures that all techniques and practices carried out during the surgical procedure are accurate and reflect standards of perioperative practice. Maintains sponge, needle and instrument counts according to policies and procedures, including notifying the surgeon of the result of the counts. Identifies, labels, cares for and handles all specimens and cultures according to established policy and procedure. Accurately interprets arterial blood gases, pulse oximetry, and clinical features of impaired gas exchange and/or airway clearance. Demonstrates understanding hemodynamic monitoring procedures, sets up equipment, assists with arterial artery and central vein cannulation. Safely administers vasoactive medications demonstrating understanding of dosage, action, use, side effects, and titration parameters associated with maintenance of critical drips. Provides ongoing assessment and monitoring of the patient in the immediate post-operative phase according to established standards and protocols. Demonstrates appropriate interventions related to potential problems/complications during the recovery phase of general, regional, spinal, or conscious sedation. Responsible for obtaining supporting information and completing required documentation related to medication reconciliation for use by the Attending MD. Responsible for implementing components of the VAP bundle consistently and per protocol. Responsible for alerting the Nurse Manager/Director/Nursing Supervisor if there is a deviation from VAP bundle by Attending MD for case review and follow up as indication. Participates actively in daily multi-disciplinary rounds for planning, coordinating, implementing and evaluating patient care.
