Provides communication/visits peri-operatively to the patient’s family and or significant other. 8. Demonstrates self-direction above and beyond daily assignments and actively offers assistance to colleagues both within the main OR’s, cardiac OR’s and various other surgical services units. 9. Demonstrates ability to accurately and thoroughly complete peri-operative nursing record documentation, i.e. surgical site confirmation, patient identification verification. 10. Brief summary of purpose: The RN Nurse Case Manager (NCM) is an integral part of an interdisciplinary team focused on care coordination, care management and improving access to and quality of care for NaviCare members. The NCM seeks to establish face-to-face and telephonic relationships with the member/caregiver(s) to better ensure ongoing service provision and care coordination, consistent with the member specific care plan developed by the NCM and Care Team.
