Members of the team may be identified on an as needed basis determined by the specific reasons for readmissions. Interacts collaboratively with all departments to foster development of readmission prevention strategies. Acts as a department resource for high complexity readmission issues. Performs concurrent chart audits for readmitted patients to identify opportunities to prevent additional readmissions. Preforms retrospective chart audits to identify causes of readmission and determine potentially avoidable causes. Interacts with physicians and other healthcare team members when indicated to facilitate appropriate patient transition of care. Maintains current knowledge in the field by attending seminars, continuing education courses and workshops. Collaborates with medical providers, patient care staff and management in the planning and implementation of patient and staff education. Job Description Nemours is seeking an Associate Nurse Manager, Staffing Resource Pool, to join our Nemours Children's Health team in Wilmington, DE. This is a full-time position, working 60% on evening and night shifts. The Associate Nurse Manager (ANM) position is responsible for collaborating with the Nurse Manager (NM) in all aspects of operational and administrative support of nursing associates and expanding nursing leadership presence in the practice area.
