Eligible for a Sign-On Bonus up to $15,000! The Infusion Nurse functions as an interactive resource in clinical nursing and professional practice. The Registered Nurse is responsible for the coordinating of and performing direct nursing care for patients, utilizing concepts of the nursing process. The Family Connects model is an evidence-based and successfully demonstrated program that connects parents of newborns to the community resources they need through postpartum nurse home visits. The model was developed in 2008 through a partnership with Duke University, the Center for Child & Family Health (CCFH), and the Durham County Health Department. As a community-based program, Family Connects supports new parents in caring for their newborn(s), offers physical assessments of the mother and the baby, addresses questions about caring for the newborn(s), identifies parents’ needs, and helps to identify community services or resources that can meet those needs. In addition, through this connection to resources, Family Connects helps to identify gaps in critical community-wide resources with the goal of working toward increasing needed services locally. A Family Connects Home Visiting Nurse provides home visiting to all families with newborns in Chicago. The goal is to support and assess family needs in 12 domains of physical and psychosocial well-being and facilitate referrals and connections to community agencies to support families in these areas. The Home Visiting Nurse will work in a team environment of clinical and non-clinical staff in order to schedule visits, and support families in the community using the evidence-based Family Connects home visiting protocol. In addition to home visiting responsibilities, the Home Visiting Nurse will support the Prenatal Education Department by teaching a minimum of one in-person prenatal education class per week, as assigned. Apply working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assist in creating a health plan for entire family. Maintain eight open visit slots and conduct and document at least six home visits per week. Respond to client needs through flexible scheduling of home visits, including occasional home visits during evening or weekend hours. Apply effective time-management skills, organization, and multitasking skills. Documentation is expected to be done and letters sent out within 48 working hours of the visit. We’re hiring for a Staff Nurse (RN) in the PICU at the University of Chicago Medicine in Hyde Park, Illinois! A Staff Nurse is a registered professional nurse (RN) who manages patient centered care that is outcome directed. Delivers high level patient care to critically ill, or injured, neonatal, pediatric, and adult patients throughout all phases of transport in collaboration with the UCAN Flight Physician, as well as Specialty Teams. Provides thorough patient assessment, collecting key information from referring medical personnel, patient, and family. Determines nursing diagnosis and priorities for care with special consideration given to the potential effects of altitude physiology and stressors of flight. Performs all post-transport activities as required, or as established under the guidance of the Administrative Director/Chief Flight Nurse, or Program/Medical Director, in collaboration with the Pilot, Flight Physician, and Communications Specialist. Assures that the patient, family, or legal guardians properly complete all authorization forms, both legal and financial. Assures that call-backs to the referring institution are completed and documented within the appropriate time frames. Maintains positive, as well as effective, communication and interaction at all levels. Maintains confidentiality in matters relating to patient/family. Provides information to patients/families which reduces anxiety and conveys UCH values of patient centered care. Actively participates in the UCAN Quality Assurance/Continuous Quality Improvement process. Actively participates in the education process of both UCH and outside referring medical communities on UCAN services, as well as specific presentations relating to diagnosis/age specific patient management. Actively participates in UCAN Committees. Supports and identifies areas of UCAN cost containment. Identifies areas for professional growth and seek opportunities for development. Completes required clinical hours in clinically related areas as mandated by IDPH and UCAN guidelines. Performs other duties as requested by senior management. Required Qualifications Bachelor's Degree preferred, although equivalent previous experience as a Flight Nurse will substitute. Associate of Science in Nursing or higher Current Illinois licensure as a Registered Nurse BLS through American Heart Association ACLS, PALS, NRP, TNS or TNCC, ENPC , PHTLS, or BTLS required at time of hire, or within 6 months of employment CFRN required within one year of hire.
