Experience in Home Care, case management, or long-term home care. Demonstrated skills in planning, organizing and directing the work of others required. Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments. Proficient with DOH UAS assessment instrument.
