Path Assist is our tech-enabled Community Health Worker program for addressing HRSN utilizing an evidence-based, structured intervention. Role Overview: This community-focused, Registered Nurse (RN) will travel to members' homes across Massachusetts to complete in-person health assessments for a managed care health plan's dually eligible members. Purpose of Position: The primary purpose of this position is to supervise the day-to-day nursing activities of the unit during your tour of duty. Duties and Responsibilities: Meet the nursing and rehabilitation needs of each resident. Maintain current resident care plans on all residents under your care. Render and/or assist with complete nursing care, treatments, dressings, and other physician orders. Transcribe physician's orders promptly and accurately. Review patient records daily to ensure accuracy and completeness. Document comprehensive and complete nursing notes. Visit each resident daily to evaluate resident needs.
