Partners with patients and families to develop, implement and evaluate plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Assesses and incorporates evidence-based standards of practice. Collaborates with interdisciplinary team in the coordination care. Facilitates safe patient transitions to the next care setting as part of plan of care. Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care. Focuses on quality outcomes of patient care when addressing customer’s needs to include current evidence based practice and quality measures. Demonstrates understanding of diverse patient care populations and ability to modify education method to ensure patient/family comprehension. Educates by assuming responsibility to individualize and coordinate patient education, and discharge specific education that includes family/caregiver. Evaluates for patient comprehension and documents. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. DEPARTMENT DESCRIPTION: S1400 is a 24-bed acute care nursing unit that provides care to medical patients that may or may not require telemetry monitoring. This unit cares for a wide variety of medical issues, including Alzheimer’s/dementia, psych, sepsis or other advanced infections, fall/syncope, palliative/ hospice, stroke, COVID-19, cardiac diagnosis: atrial fibrill ation, congestive heart failure, acute coronary syndrome, etc. Our unit population consists of patients ages 20 to geriatrics. In addition to inpatient status, the staff also monitors and cares for patients who have been placed in an observation or outpatient extended stay status.
