Job Summary Provides direct and indirect patient care in the office setting to patients, families and caregivers in the Medical Group practice. Uses the nursing process consistent with acceptable standards of the profession and MVHS Medical Group. Provides high quality patient centered care that reflects initiative, flexibility and responsibility with a minimum amount of administrative supervision. Determines priorities of care based on physical and psychosocial needs, as well as factors influencing patient flow through the system. Communicates with outpatient clinic physicians and NP/PAs (healthcare providers) about changes in patient’s status, symptomatology and results of diagnostic studies. Is able to respond quickly and accurately to changes in condition or response to treatment. Core Job Responsibilities • Support the PCP in the delivery of care by gathering and documenting relevant clinical data, as appropriate and per patient need. • Assists in specialty office procedures as directed. • Participate in the daily huddle to identify individual patient needs for upcoming appointments. • Clinical liaison between patient and provider for questions, concerns and testing results. • Communicate effectively with other members of the care team in coordinating the individualized plan of care. • Support PCP during in office procedures and examinations. • Provide outreach and education to patients in need of services as identified through quality improvement initiatives (Population Health Management). • Perform duties within scope of practice to the highest level of licensure. • Provide direct support & guidance to other members of the care team as appropriate. • Able to perform assessment data collection on all patients utilizing tools / forms in EMR. • Adequately assess and reassess pain. Educate the patient and family regarding pain management. • Provide age appropriate care to patients in the community setting. • Consistently document procedures completed in the EMR, to include medications administered and follow-up assessments • Deliver medication utilizing the five (5) rights and observes patient for desired untoward effects. • Maintain current knowledge of medications and their correct administration. • Establish priorities of patient care based on needs and office resources. • Demonstrates the ability to assist providers with procedures and perform services requiring technical and manual skill • Perform other duties as required. Education/Experience Requirements Required: • Graduate of an accredited school of nursing or nursing program. • Current New York State Registered Nursing registration. • BLS Certification. • Minimum 3 years of nursing experience • Basic computer and EMR knowledge. • Excellent communication and interpersonal skills. • Ability to assimilate information from many different sources. • Must be able to communicate in English both verbally and in writing.
