Acts an educational resource for wound care including ostomy care related issues for hospital staff and physicians. Confers with patients and families to address discharge needs i.e. regimen of home treatment, and other details of required care (referral to other facility or healthcare provider). Works to promote continuity of patient care with WHC, home health agencies, and other hospitals. Demonstrates knowledge of principles of wound care/ostomy care through education of patient and family in wound care, dressing applications, pressure ulcer care, compression therapy, diabetic foot care, venous disease, off-loading, and underlying disease processes. Facilitates departmental operations through daily completion of all required documentation, all patient charges and other paper work.
