Overview The Wound and Ostomy Nurse is responsible for the assessment, planning, implementation, and evaluation of care for the patient�s alterations in health related to wound, ostomy, and skin integrity needs in collaboration with the primary nurse. Provides direct patient care and education to selected patients and serves as a resource and educator to the staff nurse. Is an integral member of the treatment team and is responsible for the case management of specific patients, working autonomously and collaboratively with the health care team. Evaluates interventions as needed to achieve optimal patient outcomes. Whenever possible, the patient, the family, or those significant to the patient are an integral part of the assessment, planning, intervention and evaluation efforts of the Wound and Ostomy Nurse. The Wound and Ostomy Nurse provides clinical support to achieve the organization�s mission of excellent patient care and nursing�s vision of setting a new standard of excellence in autonomous and accountable nursing practice committed to patient advocacy and innovative patient care in a climate of trust and collaboration. Qualifications Minimum Education and Experience Requirements: BSN required. Current staff without BSN must obtain degree by December 2012 or as contracted by the VP/AVP. Professional nursing board certification in specialty area required within two years of specialty experience or certification eligibility date. Minimum of three years experience in nursing with relevant clinical experience in assigned patient population required. Motor Vehicle Operator Designation: Employees in this position: Will not operate vehicles for an assigned business purpose NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job. Authorizes prescription refills as instructed by the provider. Assists with test tracking and monitoring overdue results. Removes sutures and changes dressings on wounds as necessary. Participates in daily huddles with the provider. Serves as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors and colleagues. Communicates with all patients, visitors and colleagues in a professional manner while keeping customer service a top priority. Handles all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures. Performs various front office responsibilities as necessary. May require some regional travel - valid driver's license is required if traveling. Participates in the on-call responsibilities for this position as required by practice location. Perform patient and control testing on unit-specific Point of Care testing, according to Lab regulatory standards. Report results and findings to the Licensed Care worker.
