Overview Good Shepherd Hospice is seeking a compassionate and experienced Registered Nurse (RN) to provide telephone triage support for hospice patients and families. The Triage Nurse assesses patient needs, coordinates care and services, provides clinical guidance, and ensures timely interventions to support quality end-of-life care. Job Details Key Responsibilities Respond to patient, family, and caregiver calls during assigned triage hours. Assess patient needs and determine appropriate interventions. Coordinate hospice services, including medications, equipment, supplies, and nursing visits. Communicate with physicians and interdisciplinary team members regarding patient status and care needs. Provide education, support, and reassurance to patients and families. Complete accurate and timely electronic documentation. Participate in staff meetings, continuing education, and team collaboration. Qualifications: Graduate of an accredited nursing program; BSN preferred. Current New York State RN license required. Minimum of 2 years of recent experience in hospice, oncology, home health, or medical-surgical nursing. Hospice and palliative care certification preferred.
