Job Summary The Wound Care Nurse is responsible for providing specialized nursing care to patients with wounds, ostomies, or skin integrity concerns. This role includes assessment, development of care plans, implementation of treatments, patient and staff education, and documentation in accordance with clinical standards and facility policy. The nurse acts as a resource and consultant to clinical teams regarding complex wound care management and best practices. Essential Functions Provides comprehensive wound assessments and collaborates with providers on treatment plans. Implements and monitors wound care interventions including negative pressure therapy, dressing changes, and offloading techniques. Coordinates care for patients with complex wounds and ensures continuity through discharge planning and follow-up referrals. CHOP Critical Care Nurses work collaboratively with the interdisciplinary team to ensure the best outcomes using innovative technologies. The Critical Care areas are equipped to deliver therapies that are aimed toward improving survival, hastening recovery, minimizing disability, and relieving pain and suffering from critical illness and injury in a safe, humane, and respectful manner. Conditions that commonly cause critical illness and injury or require intensive care include respiratory failure, renal failure, severe infection, multi-system organ failure, trauma, extensive surgery or organ transplantation, congenital anomalies, and hematologic or immunological disorders. There are three distinct units within the Pediatric Intensive Care Unit (PICU). Unit populations are as follows: PICU South - 24 bed unit with a mixed medical and surgical population serving patients that require Intensive Care with a diagnosis within a variety of medical subspecialties including a specific subset of oncology receiving radioactive therapy. PICU Main - 26 bed unit with a mixed medical and surgical population serving patients that require Intensive Care with a diagnosis within a variety of medical subspecialties PICU West - 24 bed unit predominately focused on Neurocritical Care patients but also will care for all patients that would require pediatric intensive care. This is a full-time position; however, there may be opportunities for part-time based on unit needs at the time of application. Acute care experience (adult or pediatric) is strongly preferred. Monitor Medicare and Medicaid reimbursement opportunities while maintaining regulatory compliance. Conduct orientation and ongoing education for MDS Coordinators and interdisciplinary team members. Develop and maintain training materials, procedural guides, and MDS educational resources. Maintain current knowledge of CMS regulations, MDS updates, RAI Manual revisions, PDPM requirements, and state-specific reimbursement guidelines. Ensure regulatory changes are communicated and implemented across all facilities. Oversee CMS Targeted Probe and Educate (TPE), Additional Documentation Request (ADR), and other reimbursement audits. Analyze reimbursement trends, quality indicators, and operational metrics. Prepare weekly updates highlighting reimbursement issues, regulatory changes, best practices, and facility performance. Review facility reimbursement processes and advise Administrators and regional leadership regarding opportunities for improvement. Assist facilities during state and federal surveys and ensure survey readiness related to MDS and reimbursement.
