Overview The Wound and Ostomy Nurse is responsible for the assessment, planning, implementation, and evaluation of care for the patient�s alterations in health related to wound, ostomy, and skin integrity needs in collaboration with the primary nurse. Provides direct patient care and education to selected patients and serves as a resource and educator to the staff nurse. Is an integral member of the treatment team and is responsible for the case management of specific patients, working autonomously and collaboratively with the health care team. Evaluates interventions as needed to achieve optimal patient outcomes. Whenever possible, the patient, the family, or those significant to the patient are an integral part of the assessment, planning, intervention and evaluation efforts of the Wound and Ostomy Nurse. The Wound and Ostomy Nurse provides clinical support to achieve the organization�s mission of excellent patient care and nursing�s vision of setting a new standard of excellence in autonomous and accountable nursing practice committed to patient advocacy and innovative patient care in a climate of trust and collaboration. Qualifications Minimum Education and Experience Requirements: BSN required. Current staff without BSN must obtain degree by December 2012 or as contracted by the VP/AVP. Professional nursing board certification in specialty area required within two years of specialty experience or certification eligibility date. Minimum of three years experience in nursing with relevant clinical experience in assigned patient population required. Position Details: Qualifications Education: Graduate of an accredited registered professional nursing program Licensure: RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements. Preferred Experience: 6 months clinical healthcare experience, preferably in a primary care office.
