Implements and records interventions appropriate to diagnosis or patient problem, with no more than 3-5 exceptions, as measured by random supervisory chart review and/or direct supervisory observation. Evaluates the patient’s progress toward attainment of identified goals, with no more than 2-3 exceptions, as measured by random supervisory chart review and/or direct supervisory observation. Adheres to approved, research-based practice standards, with no more than 2-3 exceptions, as measured by direct supervisory observation and random supervisory chart review. Directs and supervises patient care provided by LPNs and support personnel 100% of the time according to department guidelines, as measured by co-worker feedback and managerial observation. Demonstrates consideration of safety, effective utilization of equipment and materials, and cost in managing patient care, as measured by direct supervisory observation and co-worker feedback. Acts effectively as charge nurse, group leader or preceptor when assigned, as determined by co-worker or preceptee feedback and direct managerial observation. Writes a progress note that demonstrates an understanding of the patient’s status and progress toward achievement of goals (80-90%). Ability to function in charge nurse or leadership capacity as determined by practice arena. Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights. Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity, 100% of the time, as measured by no significant, substantiated patient or peer complaints received by the manager/supervisor. Assures that patients have informed consent signed for indicated procedures, 100% of the time, as measured by random supervisory chart review. Acts on behalf of patients in an ethical manner 100% of the time, as measured by co-worker and patient feedback and managerial observation. Adheres to ANA Code of Ethics for Professional Nurses. Acquires and maintains current knowledge and skills in his or her area of practice. Provides proof of current licensure validation to supervisor. (To achieve excellence, one must do this in a proactive, timely manner prior to due date). Completes required unit credentialing according to departmental policy by providing documented evidence of completion to supervisor. Provides to supervisor evidence of completion of all required training and competency validation. (To achieve excellence, one must take personal responsibility for this task and complete by the due date). Participates in level appropriate number of hours of educational activity, according to pathway, related to job function annually and provides documented evidence of attendance or activity to supervisor. Participates in quality improvement processes for the department and institution as assigned and as measured by supervisory observation. Participates in nursing research. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. Coordinate resident care plan with the physical, occupational, respiratory, and speech therapy department to ensure proper care and treatment when clinically appropriate. Schedule and conduct regular conferences with the staff to discuss changes in resident's condition and problems or concerns regarding resident's care. Delegate the responsibility to daily review the documentation of dispensing controlled substances and narcotics. Reviews the records no less than weekly, or sooner if a discrepancy is noted, to ascertain that they are properly counted and documented from shift to shift.
