This full-time position provides diagnostic evaluation and medication management follow up visits for patients with psychiatric illness. The aim is to provide time limited interventions to optimize the psychiatric medication plan, reduce symptoms with the goal of transition of care to return the patient to primary care for long term management of chronic conditions. This position would be at one of our two primary locations, LGHP BH practice 802 New Holland Avenue or Suburban Outpatient Pavilion on Harrisburg Pike, reporting to the managing physician Dr. Taylor. The PMHNP would participate in our structured APP onboarding and mentorship program, which provides two hours of weekly mentorship with a psychiatrist and an hour-long weekly APP clinical collaboration meeting with other APPs, both of which support professional practice and growth. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.
