SUMMARY: The primary purpose of this job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assist in planning, developing, organizing, implementing & evaluating the activity programs & quality assurance program in this facility for the Activities Department. Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for resident. Purchase department supplies, clothing and personal items for the residents. Maintain the confidentiality of all resident care information including protected health information & follow facility guidelines in reporting non-compliance with said guidelines & treat residents with dignity at all times. Oversee residents engaged in scheduled activities to promote the physical, social, and mental well-being of residents by applying experience in activities and knowledge of diagnoses and conditions of residents. Assists in-room residents by visiting with them, writing letters, running errands, and perform assistance with crafts, projects, etc., as necessary. QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. Experience in Long Term Care Activities Department is preferred. Must have patience, tact, cheerful disposition & enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning. Must be able to read, write, speak, and understand the English language. PHYSICAL DEMANDS: The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. General Purpose The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. Essential Duties • Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. • Use only authorized abbreviations established by this facility when recording information. • Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical. • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. • Report all accidents and incidents you observe on the shift that they occur. • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator. • Report any known or suspected unauthorized attempt to access facility’s information system. • Perform only those nursing care procedures that you have been trained to do. • Ensure that the resident’s room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). • Greet residents and escort them to their room. • Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. • Make resident comfortable. • Inventory and mark the resident’s personal possessions as instructed. • Store resident’s clothing. • Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. • Transport residents to new rooms or to the receiving area. • Assist with loading/unloading residents’ to/from vehicles as necessary. • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. • Meet with your shift’s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Report all complaints and grievances made by the resident. • Participate in and receive the nursing report as instructed. • Follow established policies concerning exposure to blood/body fluids. • Make beds as instructed. • Put extra covers on beds as requested. • Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed. Report changes in the resident’s eating habits to the Nurse Supervisor/Charge Nurse. • Keep residents’ water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. • Perform after meal care (i.e., remove trays, clean resident’s hands, face, clothing, etc.). • Participate in and receive the nursing report upon reporting for duty. • Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. • Follow work assignments, and/or work schedules in completing and performing your assigned tasks. • Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. • Notify the facility when you will be late or absent from work. • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents. • Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). • Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). • Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. • Perform restorative and rehabilitative procedures as instructed. • Assist in preparing the resident for a physical examination. • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). • Report injuries of an unknown source, including skin tears. • Observe disoriented and comatose residents. August Family Birth Place : The Family Birth Place provides services to obstetric and gynecological populations. They are devoted to a flexible family centered care model. This unit has 7 labor rooms, 14 post partum rooms, 7 surgical beds, 3 triage rooms, 2 operating rooms, and a Level I nursery with a 3 bed special care nursery. ​ CLINICAL LADDER LEVEL I-IV The RN level will be determined at the time of hire based on the candidate’s qualifications and ability to meet Clinical Ladder requirements. An established compensation add-on is applied to each RN level. Level expectations include: RN Level II: Minimum 6 months RN experience; AAS in Nursing; completion of Clinical Ladder requirements within 12 months, including 3 activities per pillar (Leadership, Quality, Organizational Contribution), 9 CEUs, and 6 flex items. RN Level III: Minimum 1 year RN experience; AAS in Nursing; demonstrated clinical and leadership skills; preceptor and/or charge experience; completion of 4 activities per pillar, 12 CEUs, and 15 flex items. RN Level IV: Minimum 1 year experience in the specialty hired into; BS in Nursing required; expanded leadership role (charge, preceptor, mentor); completion of Clinical Ladder requirements within 12 months, including 5 activities per pillar, 15 CEUs, and 20 flex items. RESPONSIBILITIES : Provides high-quality, patient-centered, clinical care Performs initial and ongoing assessment of the patient, utilizing appropriate data (i.e. physical, psychological, educational and risk-screen data appropriate for the chronological / developmental age of the patient). Communicates and documents assessment of patient with the interdisciplinary team. Utilizes all available resources in order to meet the needs of the patient and family. Develops and documents plan of care, treatment and services which is individualized and appropriate for the patient needs, strengths, limitations and goals. Complies with organizational policies, procedures and protocols including policies governing safety, medication administration and documentation. Revises plan of care based on ongoing evaluations and updates documentation appropriately. Completes at least one clinical research utilization (CRU) form for submission to Nursing Research Department utilizing a research article from the last 5 years from research performed in the United States. Completes a minimum of 5 nationally approved contact hours (CNE or CEU) per year. Completes mandatory in-services and health screens in timely manner. Maintains Basic Life Support (BLS) certification. REQUIRED QUALIFICATIONS: Diploma or Associate’s Degree in Nursing Registered Nurse license in New York State. Basic Life Support (BLS) certification.
