In this role, the successful candidate carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. As a member of the interdisciplinary healthcare team, formulates the initial plan of care with the provider (physician or nurse practitioner) and collaborates with them to implement a comprehensive patient-centered plan of care to achieve expected outcomes; utilizes principles of assignment and delegation consistent with the NY State Nurse Practice Act. Job Responsibilities: Collects patient healthcare data systematically and on an ongoing basis using appropriate assessment techniques and instruments. Implements interventions identified in the plan of care. Develops a plan of care that prescribes intervention to attain expected outcomes. Identifies expected outcomes and anticipates unexpected or adverse outcomes as individualized to the patient. Analyzes assessment and reassessment data to determine nursing diagnoses. Minimum Qualifications: To qualify you must have current Registered Professional Nurse in New York. BLS Certified through the American Heart Association. Baccalaureate Degree in Nursing required with a minimum of one year Med/Surg nursing experience. Professional Organization: Membership in pertinent of professional organizations preferred. Others: Fluent written and spoken English language skills, physical stamina, manual dexterity and visual acuity to perform responsibilities. These roles are responsible for providing direct patient care, ongoing instructions and reinforcement of education and care plans, and providing general clinical support under the direction or supervision of the patient’s physician or LIP/RN designee in accordance with policy, procedure, and competency to promote patient health and wellness. This position requires providing services to all age populations in a manner that demonstrates an understanding of the functional/developmental age of the individuals served. Assist the registered nurse and/or provider through data collection concerning the physical, psychological, social, and cultural dimensions of patients according to practice standards and institutional policy/procedure Organizes and prioritizes nursing care activities considering the needs of the patients and the interdisciplinary team. Implements age-appropriate interventions based on individual patient’s needs as directed by the registered nurse/provider. Assist in evaluating the effectiveness of care given in progressing patients toward desired outcomes. Provide ongoing instructions and reinforcement of education and care plans developed by the Provider/RN based on documented health and education needs and the condition and age of the patient. Provides a safe environment and safe delivery of care. Assumes responsibility for professional development of self and contributes to and assists with the professional development of others. MINIMUM REQUIREMENTS Education: Graduated from an accredited Licensed Practical Nurse Program. Experience: No experience required. Licensure: Licensed to practice as a Practical Nurse (LPN) in the Commonwealth of Virginia prior to onboarding. American Heart Association (AHA) Health Care Provider BLS certification required. May require a valid Virginia Driver's License (VADL). PHYSICAL DEMANDS Job may require standing for prolonged periods, frequently traveling and bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease. The pay range for this role is $23.29 - $37.26 hourly.
