Job Requirements POSITION SUMMARY: The Hospice RN oversees the development, implementation and evaluation of the plan of care in collaboration with the interdisciplinary team to create the most positive Hospice experience for the patient and family. PRIMARY ACCOUNTABILITIES: Quality/No Harm: Verbalizes, demonstrates and documents nursing care, change in condition, treatments and procedures in compliance with Health First/Hospice Policy, Federal/State Regulations and National Safety Patient Goals. Ensures all patient transfer information from one level of care to the next is accurately and completely documented for utilization by the billing department to meet deadlines as per corporate finance. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed.
