Collaborates with the interdisciplinary team in planning and delivery of care to hospice patients and families. Performs other duties as assigned. Supervisory/Management Responsibilities This is a non-management job that will report to a supervisor, manager, director or executive. ​ Minimum Requirements Education - Completion of an LPN program recognized by the SC State Board of Nursing. Experience - One (1) year experience as an LPN. Experience in Hospice preferred. In Lieu Of In lieu of one (1) year of LPN experience, will accept successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency program from another organization. In lieu of completion of a LPN program, will accept program equivalency recognized by the SC State Board of Nursing. Required Certifications, Registrations, Licenses Holds a current LPN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an LPN in the state the team member is working. Description Position Summary: Provides direct and individual nursing care to assigned residents based on nursing standards and under the supervision of the Director of Resident Care (DRC). Also maintains compliance with all state and federal regulations and the standards of HRA. Essential Functions: Ensures quality and safe delivery of health services to residents Implements plan of care formulated by physician and DRC Provides accurate and timely documentation of care given Assesses and provides resident and family/care giver information pertinent to diagnosis or plan of care Administers medication per physician orders. Documents properly on MAR.
