Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Prepares for and assists with routine and specialty procedures. 7. Schedules patients for follow up appointments or referrals and performs routine clerical duties for team and staff to include photocopying, scanning and faxing. 8. Performs telephone messaging and in person screening with appropriate follow up. 9. Responds to emergency situations. 10. Respects and maintains privacy and confidentiality per HIPAA guidelines. 11. Basic Life Support (BLS) by American Heart Association (AHA) required Certified Medical Assistant (CMA) by American Association of Medical Assistants (AAMA) required or Registered Medical Assistant (RMA) by American Medical Technologist (AMT) required or National Certified Medical Assistant (NCMA) by National Center for Competency Testing required or Certified Clinical Medical Assistant (CCMA) by National Healthcareer Association (NHA) required or Minimum Education: High school diploma or equivalent Completion of an approved Medical Assistant training program OR one (1) year of experience (in addition to the three (3) years of experience required to meet the experience requirement) AND certification may be substituted for Medical Assistant training program. Minimum Experience: Three years of experience in a healthcare setting. Physical Requirements: Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
