Give resident as much assistance as required (cut meat, open containers) encourage the resident to eat and/or feeds resident as indicated on the assignment sheet. Record resident's intake and repost to nurse if resident's intake is low meal is refused, etc. 3. CLINICAL DUTIES CONTINUED: 6. Transfers residents according to method indicated on assignment sheet. Follows policies and procedure on body mechanics and use of resident lifts. Notify nurse if in doubt of resident's capabilities or questions about transfer method to be used. 7. Ambulates residents as directed by assignment sheet, Charge Nurse or Physical Therapy. 8. Obtains the resident's vital signs and weight according to shift routine or as directed by Charge Nurse. 9. Answers calla lights promptly for any resident and addresses the resident's needs 10. Responds immediately to bed/chair alarms, Code Alert or door alarms. 11. Collects specimens of urine, stool, and sputum as directed by the Charge Nurse. 12. Passes out fresh water and ice, nourishments and snacks to residents as assigned, being sure to follow the resident's diet. 4. CLINICAL DUTIES CONTINUED: 13. Ensures safety of all residents. 14. Immediately reports to Charge Nurse any unusual occurrences or injury of a resident. 15. Immediately reports hazard/malfunctioning equipment to the Charge Nurse. 16. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. ESSENTIAL POSITION FUNCTIONS A. Job Knowledge and Role Responsibilities: 1. Attends all mandatory in-services by employee’s anniversary date. 5. Acts appropriately under the direction of the Charge Nurse and acts as an active member of the interdisciplinary team. 6. Demonstrates ability to adjust to changes in units/shift assignments to meet resident and facility needs. 7. Demonstrates knowledge and ability to provide care on specialized dementia unit, as appropriate. 8. Demonstrates knowledge of and complies with the Code of Conduct and Corporate Compliance Program when performing work functions. B. Resident Care Responsibilities: 1. Answers call bells on a timely basis to respond to resident needs. 2. Completes or assists residents with bathing, dressing, hygiene, and grooming, in accordance with established Care Plan. 3. Assists residents with oral care (teeth brushing) as per Care Plan. 4. Weighs residents and records weights monthly/weekly or more frequently as assigned. 5. Serves and collects food trays, feeds residents requiring help, provides meal supplements and fresh drinking water as needed. 6. Moves or assists residents in moving to and from bed as necessary, and transports residents using wheelchairs, or assists them with walking, as per Care Plan. 7. Takes vital signs on all residents as per Care Plan including temperature, pulse, respiration and blood pressure. Notifies Charge Nurse of any changes or findings beyond normal limits. 8. Records food and liquid intake and emission as directed. 9. Reports all unusual conditions and reactions to nurse in charge. 10. Assists in keeping residents’ rooms and equipment clean, neat and orderly. Cleans and maintains therapeutic equipment (i.e. nebulizers, oxygen concentrators, IV poles and pumps, wheelchairs, gerichairs, siderail pads, linen carts, med carts, treatment carts, tub rooms, etc.). Dates and labels all personal hygiene items. Removes and cleans all therapeutic equipment and returns to its appropriate storage area if a resident is discharged or expires. 11. Changes bed linen and incontinent care items as needed. 12. Follows established systems: a) Releases restraints and repositions residents according to established Care Plan. b) Ensures that all residents are out of bed according to established Care Plan (if appropriate). If a resident does not get out of bed, notifies Charge Nurse. c) Turns, positions and toilets residents as per Care Plan. d) Ambulates residents according to the plan of care and facility policy. C. Role Responsibilities-Interpersonal Skills: 1. Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information. 2. Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner. 3. SUMMARY: Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA’s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job. SPECIFIC DUTIES AND FUNCTIONS: The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA. The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI. The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting. The CLA performs the care or services that are outlined in the individual participant’s care plan; any care guidelines that are pertinent to the participant’s diagnosis are specified in the care plan. At this time the participant’s satisfaction with the Care Plan and CLA is evaluated Driving is required within PACE SEMI catchment area The CLA must have reliable transportation available on a daily basis, a current valid driver’s license, must maintain an acceptable driving record, and provide proof of current automobile insurance The CLA must have the ability to transfer and assist with moving patients; frequent walking, bending, and lifting of forty (40) pounds or more may be needed in the performance of duties The CLA may be required to change participant assignments based on the changing acuity level, participant needs, and participant census.
