About this Job: General Summary of Position Functions as a member of the patient-centered care team to provide direct patient care activities under the supervision of a Registered Nurse for assigned patients and their families/significant others. Assists medical and nursing staff with procedures and/or tests and patients with Activities of Daily Living (ADLs). Accurately takes and records patient's vital signs height and weight pulse oximetry point of care and waived testing (i.e. finger sticks for glucose monitoring/Accucheck urine dipstick hemoccult) and other pertinent data or information in the patient's health record. Performs routine sterile non-sterile (e.g. application of thermal soaks) and other treatments/activities identified by Unit/Hospital policy inclusive of but not limited to: tube feedings; ostomy care; Foley Catheter care; perineal care; simple dressing changes and wound care; turning and positioning; postmortem care and transportation of body to the morgue; and assistance with the application maintenance and removal of traction casts splints and exercise equipment. Maintains the comfort privacy and confidentiality and dignity of each patient in the delivery of services. Performs patient observation duty including 1:1 observation and safe application of restraints. Prepares patients to be seen by providers. Assists providers and registered nurses in patient examinations and procedures. Rounds on all assigned patients based on established standards (e.g. hourly purposeful rounding). Reports actual and potential problems concerning patients/families including changes in the patient's condition to the registered nurse and other care team members as necessary. Ensures complete hand off to oncoming shift and obtains full hand off from outgoing shift. Initiates and assists in CPR when indicated. Participates in patient/family teaching as directed and responds to questions from patients families and visitors in a prompt courteous and respectful manner referring them to appropriate personnel if necessary. Maintains the cleanliness and safety of patient rooms/environment by distributing and changing linen/towels removing used meal trays checking bed siderails and safety restraints etc. Assists with environmental services support such as linen management trash removal and cleaning patient rooms as needed. Transports and/or escorts patients during discharges transfers and/or medical appointments. Completes errands such as delivering specimens to the laboratory equipment and supplies etc. as required. Receives and stocks equipment and supplies and maintains orderly supply and utility rooms. Attends staff meetings ongoing educational offerings and participate in multidisciplinary quality and service improvement teams. Minimal Qualifications Education High School Diploma or GED or equivalent required Completion of an approved nursing assistant certification program required One year of relevant education may be substituted for one year of required work experience. The Certified Home Health Aide plays a critical role as a member of the Interdisciplinary Group Team. You will work under the supervision of a Registered Nurse and perform various services for patients to meet their personal needs and promote comfort. On an Average Day You Will: (includes but not limited to) Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality Meet safety needs of patient and use equipment safely and properly (foot stools, side rails, O2 etc.) Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned Assist in dressing and undressing patients, as assigned Plan and prepare nutritious meals, including shopping, as assigned Assist in feeding the patient, as assigned Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence Assess and report on patient's condition and significant changes to the Case Manager; also, being aware of the caregiver or other individuals living with the patient and interpersonal issues Assist in ambulation and exercise as instructed by the hospice nurse or therapist Provide normal range of motion and transfers/positioning, as directed Assist with self-administered medications as allowed by state regulations, such as “reminders” Offer and assist with bedpan and urinal aid, as assigned Aid with light laundry needs if necessary Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency Provide respite for patient's family/caregiver when on-site, as appropriate Maintain patient's living area is clean and orderly, as assigned Adhere to the organization's documentation and care procedures and standards of personal and professional conduct Participate in quality assessment performance improvement teams and activities Respect patient and family/caregiver environment and patient's personal needs All other duties as assigned Requirements: Must be a Certified Nursing Assistant ( applicable for all States, additional requirements for California residents, see below). CALIFORNIA REGULATION : Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California.
