Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs. Qualifications Must meet one of the following requirements. 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); 2) have one year experience within the last five years in direct client care in a home health or hospice setting. Must be registered and listed in good standing on the nurse aide registry in the state of employment. Must have basic demonstrated technology skills, including operation of a mobile device. Job Summary The Technical Partner provides direct and indirect patient care under the direction of the RN or LPN. Essential Job Functions Deliver direct and indirect patient care in accordance with hospital policies, procedures, protocols and standards of care. Perform and/or assist with patient care needs as directed and assists patient with activities of daily living. Respond to call lights, inquiries and requests from patients and families in a timely manner. Collect, document and report ongoing patient care data as directed. May perform the following skills: obtain vital signs, apply heating pads, set up suction equipment, utilize E cylinders, apply antiembolic hose, aseptic techniques, distribute and maintain sufficient supply of linens, apply Sequential Compression Devices, perform blood glucose testing, perform ostomy care, simple dressing changes, apply telemetry leads, bladder scanning, preventative skin care. Apply pulse oximetry, urinary catheter care, transport blood and blood products from blood bank, perform EKGs, oral pharyngeal suctioning, place patient in CPM machine, prepare and maintain isolation boxes and supplies, declutter patient rooms and storage areas, deliver specimens to lab, crutch measurement and teaching, maintenance of specialty carts, supply inventory and special order supplies, stock warmers with blankets, saline, and water. Make regular rounds to address patient needs and physical environment - purposeful hourly rounding, Assist in the educational process to promote and maintain health, foster self-care, and improve outcomes. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
