Job Description JOB SUMMARY Under the supervision of the UCSF Health Primary Care Administrative Nurse Manager, guided by policies and procedures of the Ambulatory Services, the Hospital Assistant III assists in the delivery of care to patients at UCSF Primary Care. Hospital Assistant III will be primarily responsible for the members of their primary care team as well as providing back up support to other teammates. The Hospital Assistant III will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment. This position floats between the front and the back office performing both clerical and clinical duties. Work includes a variety of personal care and nursing care procedures including assuring positive care outcomes. S/he will perform assigned patient care and laboratory procedures in support of services provided to patients within the practices. S/he is required to learn and be able to assist in all functions of the practices to include back and front office. The Hospital Assistant III consistently projects a professional and courteous demeanor, helps establish and maintain the highest service standards for the practice and complies with the Medical Center House and Telephone Standards. Qualifications REQUIRED QUALIFICATIONS Certification from an approved Medical Assistant program or equivalent combination of training and experience. One year experience as a Medical Assistant in a medical practice. Knowledge and abilities essential to the successful performance of the duties assigned to the position. Current American Heart Association CPR Certification. Certification in phlebotomy (CPT) Demonstrated computer familiarity, keyboard skills, computer literate in programs such as Microsoft Word and Excel. Willingness to learn new computer programs as needed to sustain practice operations. Ability to work effectively with people from diverse cultures and diverse socioeconomic situations. MDS COORDINATOR- Exempt (Salary) Position Reports to Administrator OVERVIEW: MDS Coordinator administers patient assessments and overseas the assessment process, setting the assessment schedules and assuring that assessments are done in an accurate and timely manner. The MDS Coordinator coordinates the care plan according to regulatory requirements. Ensures that resources are made available to patients and patient care is delivered effectively and to a satisfactory standard. Creates the schedule for all Medicare and Medicaid. Start Medicare coverage for newly qualified patients and remain updated on changes in Medicare coverage and help determine documents needed for reimbursement. Proactively communicates with Administrator and Dircetor of Nursing to identify regulatory risk that allow capture of resources provided on the MDS, and clinical trends that impact resident care. Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Medicaid, and Managed Care. Ensures timely electronic submission of all MDS to the state database. Reviews state validation reports and ensures that the appropriate follow up action is taken. Facilities the Care Management Process engaging the resident, IDT, and family in timely identification and resolution of barriers to discharge resulting in optimal resident outcomes and safe transition to the next care setting. Directly educates or provides company resources to the IDT members to ensure thay are knowledgeable of the RAI process. Analyzes QI/QM data in conjunction with the IDT members to identify trends. QUALIFICATIONS: Proficiency in MDS 3.0. Demonstrating knowledge of state and federal regulations. Registered Nurse or LPN with current, active license in the state. Minimum two years of clinical experience in LTC setting. PHYSICAL REQUIREMENTS: This position is very active and requires standing for long periods of time, walking throughout facility, bending, kneeling, stooping all day. Responsibilities New-graduate nurses will initiate professional nursing practice careers in a supportive environment committed to mentoring, coaching and learning.
